磁共振辅助诊断膝关节前交叉韧带损伤价值的Meta分析

Magnetic resonance imaging in the diagnosis of knee anterior cruciate ligament injury: a Meta-analysis

目的评估磁共振辅助诊断膝关节前交叉韧带损伤的临床价值。方法检索中国生物医学文献数据库和中国知网等数据库有关磁共振辅助诊断膝关节交叉韧带损伤的研究,检索时限为2010年1月至2017年12月。结果共纳入10个研究,Meta分析结果显示,特异度(SPE)合并=0.95[95%CI(0.88,0.98)],阳性似然比(+LR)=2.3[95%CI(1.0,5.5)];敏感度(SEN)合并=0.59[95%CI(0.25,0.87)],阴性似然比(-LR)=0.09[95%CI (0.04,0.18)];诊断比值比(DOR)合并=26 [95%CI (8,89)];ROC曲线下的面积(AUC)=0.94 [95%CI (0.91,0.95)]。结论磁共振辅助诊断前交叉韧带损伤具有较好的临床价值,但结论尚需高质量的前瞻性研究进行验证。

Objective To assess magnetic resonance imaging in clinical diagnosis of knee anterior cruciate ligament injury.Methods CBM and CNKI were searched systematically for studies about magnetic resonance imaging in clinical diagnosis of knee anterior cruciate ligament injury from 2010 January to 2017 December.We evaluate the quality of the included publications by the Newcastle-Ottawa Scale.Results Ten studies were included.Meta-analysis showed that SPE-merge=0.95[95%CI(0.88,0.98)];SEN-merge=0.59[95%CI(0.25,0.87)];+LR=2.3[95%CI(1.0,5.5)];-LR=0.[95%CI(0.04,0.18)];DOR=26[95%CI(8,89)];AUC=0.94[95%CI(0.91,0.95)].Conclusion Magnetic resonance imaging(MRI)is valuable in the diagnosis of anterior cruciate ligament(ACL)injury.However,high quality prospective studies are needed.
