等离子针状电极剜切治疗非肌层浸润性膀胱癌的效果

Effect Plasma Needle Electrode Enucleation for Non-muscular Invasive Carcinoma of Urinary Bladder

目的探究等离子针状电极剜切治疗非肌层浸润性膀胱癌的治疗效果,为临床实践提供理论依据。方法选取2016年4月~2019年4月辽阳泌尿外科医院收治的非肌层浸润性膀胱癌患者80例,按照随机数字表法分为参照组与研究组,每组40例。参照组采用尿道双极等离子环状电极肿瘤切除术,研究组采用尿道双极等离子针状电极剜切术治疗,比较两组治疗情况、组织病理标本保留肌层率、并发症发病率、生活质量、复发情况。结果研究组术中出血量、导尿管留置时间、住院时间均优于参照组,差异有统计学意义(P<0.05)。研究组组织病理标本保留肌层率高于参照组(10.00%vs 77.50%),差异有统计学意义(P<0.05)。研究组术后并发症发病率低于参照组(5.00%vs 20.00%),差异有统计学意义(P<0.05)。研究组术后生活质量评分高于参照组,差异有统计学意义(P<0.05)。研究组术后1年复发率低于参照组,差异有统计学意义(P<0.05)。结论非肌层浸润性膀胱癌患者实施尿道双极等离子针状电极剜切术治疗有助于减少术中出血量,缩短治疗时间,有效改善患者的生活质量,且该术式远期疗效良好,有助于降低复发率,治疗效果确切。

Objective To explore the therapeutic effect of plasma needle electrode enucleation for non-muscular invasive carcinoma of urinary bladder,to provide a theoretical basis for clinical practice.Methods A total of 80 patients with non-muscular invasive carcinoma of urinary bladderadmitted to Liaoyang Urological Hospital from April 2016 to April 2019 were selected and divided into a reference group and a study group according to a random number table,with 40 cases in each group.The reference group used urethral bipolar plasma ring electrode tumor resection,and the study group used urethral bipolar plasma needle electrode enucleation.The treatment conditions,the rate of histopathological specimen preservation,the incidence of complications,the quality of life,and the recurrence of the two groups were compared.Results The amount of blood loss,catheter indwelling time,and length of stay in the study group were better than those in the reference group,the difference was statistically significant(P<0.05).The muscle layer retention rate of histopathological specimens in the study group was higher than that in the reference group(10.00%vs 77.50%),the difference was statistically significant(P<0.05).The incidence of postoperative complications in the study group was lower than that in the reference group(5.00%vs 20.00%),the difference was statistically significant(P<0.05).The postoperative quality of life score of the study group was higher than that of the reference group,the difference was statistically significant(P<0.05).The 1 year postoperative recurrence rate in the study group was lower than that in the reference group,the difference was statistically significant(P<0.05).Conclusion Patients with non-muscular invasive carcinoma of urinary bladder undergoing urethral bipolar plasma needle electrode enucleation can help reduce intraoperative blood loss,shorten treatment time,and effectively improve the patient's quality of life.The long-term effect of this procedure is good.It helps to reduce the recurrence rate,and the treatment effect is definite.
