腹腔镜阑尾切除术阑尾残端不同处理方法效果比较

Effects comparison of different handling methods for appendiceal stump after laparoscopic appendectomy

目的分析腹腔镜阑尾切除术阑尾残端不同处理方式的临床治疗效果。方法选取2019年1月至2020年1月在本院行腹腔镜阑尾切除术的患者78例作为研究对象,依据阑尾残端处理方式的不同分为双重丝线结扎组(A组)与结扎+荷包缝合包埋组(B组),各39例,比较两组手术及恢复情况。结果两组术中出血量、住院时间及胃肠道功能恢复时间比较差异无统计学意义,A组手术时间短于B组,手术费用低于B组,差异有统计学意义(P<0.05);B组并发症发生率低于A组,差异有统计学意义(P<0.05)。结论针对腹腔镜阑尾切除术的患者双重丝线结扎和结扎+荷包缝合包埋均可行,但使用结扎+荷包缝合包埋方式处理阑尾残端,安全性相对更高,但在临床可根据患者的经济基础及临床医师的操作娴熟度选择适宜的处理方式。

Objective To analyze the effects of different handling methods for appendiceal stump after laparoscopic appendectomy.Methods 78 patients of undergoing laparoscopic appendectomy from January 2019 to January 2020 in our hospital were selected as reseach subjects,according to different handling methods for appendiceal stump,they were divided into double silk ligation group(group A)and ligation plus purse string suture and embedding group(group B),with 39 cases in each group.The operation conditions and postoperative rehabilitations between two groups were compared.Results There was no statistically significant difference in the intraoperative bleeding volume,hospital stays and gastrointestinal function recovery time between two groups.The operation time of group A was shorter than that of group B,and the operation cost was lower than that of group B,the difference was statistically significant(P<0.05);the complication rate of group B was lower than that of group A,and the difference was statistically significant(P<0.05).Conclusion Both of double silk ligation and purse string suture and embedding have fair effects for treatment of appendiceal stump.However,ligation combined with purse string suture and embedding can increase the surgical safety.Therefore,the proper processing methods may be flexibly chosen according to patient's economic burdens.
