血管超声检查在缺血性脑血管疾病患者颈动脉狭窄诊断中的应用价值

Application value of vascular ultrasound examination in diagnosis of carotid artery stenosis in patients with ischemic cerebrovascular disease

目的探讨血管超声检查在缺血性脑血管疾病患者颈动脉狭窄诊断中的应用价值。方法选取2017年2月~2019年3月沈阳市第二中医医院收治的71例缺血性脑血管疾病患者作为研究对象,所有患者均进行血管超声检查,并以脑血管造影检查结果为金标准,分析血管超声检查应用于缺血性脑血管疾病患者颈动脉狭窄诊断中的价值。结果经脑血管造影诊断确诊,71例缺血性脑血管疾病患者中检出轻度颈动脉狭窄15例,中度颈动脉狭窄45例,重度颈动脉狭窄8例,完全闭塞3例。血管超声检查诊断缺血性脑血管疾病颈动脉狭窄的准确性为88.73%(63/71)。经Kappa一致性检验,血管超声检查诊断缺血性脑血管疾病颈动脉狭窄的结果与脑血管造影诊断结果的一致性较好(Kappa=0.798,P=0.000)。受试者工作特征(ROC)曲线显示,血管超声检查诊断缺血性脑血管疾病颈动脉狭窄的曲线下面积(AUC)为0.956,诊断价值很好,其敏感度及特异性分别为0.933、0.046。结论血管超声检查用于缺血性脑血管疾病患者颈动脉狭窄诊断中的价值较为显著,且和脑血管造影诊断结果的一致性较好。

Objective To explore the application value of vascular ultrasound examination in the diagnosis of carotid artery stenosis in patients with ischemic cerebrovascular disease.Methods A total of 71 patients with ischemic cerebrovascular disease who were admitted to Shenyang Second Hospital of Traditional Chinese Medicine from February 2017 to March 2019 were selected as the research objects.All of the patients underwent vascular ultrasound examination.Cerebrovascular angiography results were used as the gold standard.The application value of vascular ultrasound examination in the diagnosis of carotid artery stenosis in patients with ischemic cerebrovascular disease was analyzed.Results According to cerebral angiography results,15 patients were diagnosed with mild carotid stenosis,45 patients with moderate carotid stenosis,8 patients with severe carotid stenosis and 3 patients with complete occlusion among the 71 patients.The accuracy of vascular ultrasound examination in the diagnosis of carotid artery stenosis in patients with ischemic cerebrovascular disease was 88.73%(63/71).The consistency of diagnosis results of vascular ultrasound examination and cerebrovascular angiography in the diagnosis of carotid artery stenosis in patients with ischemic cerebrovascular disease was good based on the Kappa consistency check(Kappa=0.798,P=0.000).The receiver operating characteristic(ROC)curve showed that the area under curve(AUC)of vascular ultrasound examination in the diagnosis of carotid artery stenosis in patients with ischemic cerebrovascular disease was 0.956,indicating good diagnostic value.The sensitivity and specificity were 0.933 and 0.046.Conclusion The application value of vascular ultrasound examination in the diagnosis of carotid artery stenosis in patients with ischemic cerebrovascular disease is significant,and has good consistency with the diagnosis result of cerebral angiography.
