摘要
阿霉素广泛用于肿瘤的化疗,但可引起剂量依赖性的心脏毒性。阿霉素引起心脏毒性的机制复杂且具有不可逆性,与其增加氧化应激、抑制拓扑异构酶2β、干扰线粒体功能等有关。在保证阿霉素抗肿瘤作用下减轻其心脏毒性是肿瘤化疗的研究重点之一。蒽环类似物、神经激素拮抗剂、他汀类药物等可减轻阿霉素的心脏毒性。本文就阿霉素引起心脏毒性的机制、监测方法及治疗的研究进展作一综述。
Doxorubicin has been widely used in tumor chemotherapy,but it could cause dose-dependent cardiotoxicity.The doxorubicin-induced cardiotoxicity has complex mechanism and is irreversible.It is correlated with the increase in oxidative stress,inhibition of topoisomerase 2β,and interference with mitochondrial function.Under the premise of ensuring the anti-tumor effect of doxorubicin,to reduce its cardiotoxicity is one of the key points of tumor chemotherapy.Current studies have shown that anthracycline analogs,neurohormone antagonists,statins and other drugs can reduce the cardiotoxicity of doxorubicin.This paper reviews the research progress of the mechanism of doxorubicin-induced cardiotoxicity,its monitoring methods and treatment.
