股前外侧KISS皮瓣在头皮和女性外阴肿瘤切除修复的效果

Efficacy of anterolateral thigh flap with KISS technique on reconstruction of scalp and vulvar tumors

目的探讨股前外侧KISS皮瓣修复头皮及女性外阴皮肤恶性肿瘤切除术后缺损的方法及效果。方法2016年3月至2019年3月,华中科技大学附属协和医院整形外科收集肿瘤患者32例,女24例,男8例,年龄18~68岁,平均49.4岁。对15例头皮恶性肿瘤行扩大切除及游离股前外侧KISS皮瓣转移修复,对17例外阴恶性肿瘤患者行外阴肿物扩大切除及带蒂股前外侧KISS皮瓣转移修复。结果15例头皮修复及17例外阴修复患者皮瓣全部完全成活,头皮修复患者中2例出现缝合口愈合不良,外阴修复患者中2例发生皮瓣尖端部分坏死,4例出现缝合口感染,2例出现皮下通道血肿。所有皮瓣供区未出现并发症,术后头皮及外阴形态及功能满意。结论股前外侧KISS皮瓣具有供区损伤小、修复效率高、皮瓣血供可靠、术后恢复快的优点,在修复头皮及女性外阴较大面积皮肤恶性肿瘤切除术后缺损中有一定的应用价值。

Objective To explore the effect of the anterolateral thigh flap with KISS technique for reconstruction of scalp and vulva.Methods Fifteen patients with scalp tumor and 19 patients with vulvar tumor were treated with radical resection and transfer of the anterolateral thigh flap with KISS technique from March 2016 to March 2019.Results Wound dehiscence was observed in 2 cases of scalp reconstruction.Part necrosis of flap was observed in 2 cases of vulvar reconstruction,4 cases had wound dehiscence and 2 cases had hematoma.No total failure of flap was observed.No donor-site complication was observed.The contour and function of reconstructed vulva were satisfactory.Conclusions The anterolateral thigh flap with kiss technique for reconstruction of scalp and vulva is reliable.It features low donor-site morbidity,high reconstructive efficiency,reliable perfusion and quick postoperative recovery.
