Experience of professor DU Kekuan’s differential treatment of pediatric diseases from five zang-organs and three-layers syndrome differentiation method

杜克宽教授针对小儿生理病理特点,认为小儿疾病主观症状并不多,小儿疾病宜从趋势的角度入手辨病机,执两端而和其中,使治疗的方向不失偏颇。八纲中的阴阳、表里、寒热、虚实辨证就是趋势辨证。杜教授临床从五脏三层辨证法论治儿科疾病,即五脏辨证主要是症状的辨证,运用脏腑学说,通过辨析望闻问切四诊信息,用变异和正常征象比较,以诊断测知五脏病变的部位。寒热辨证是辨别疾病的性质,机体外感六淫之邪,内应于五脏六腑,则病必有寒热之别。虚实辨证可以了解机体的邪正盛衰,实证宜攻,虚证宜补,为治疗提供理论依据。附典型案例1则,以资验证。

Aiming at the pathophysiological characteristics of children,professor DU Kekuan holds that the subjective symptoms of children’s diseases are not many.It is advisable to differentiate the pathogenesis of children’s diseases from the perspective of the trend,and hold both ends and harmonize them,whereby the direction of treatment is not biased.The eight principles of yin and yang,exterior and interior,cold and heat,deficiency and excess syndrome differentiation is the trend of syndrome differentiation.Professor DU treats pediatric diseases from the five zang-organs and three-layers syndrome differentiation,that is,the syndrome differentiation of the five zang-organs is mainly the syndrome differentiation of symptoms.By using the theory on zang and fu organs,through the differentiation and analysis of the four diagnostic information,and by comparing the variation and normal signs,the location of the disease in the five zang-organs can be diagnosed and detected.Syndrome differentiation of cold and heat is differentiating the nature of the disease.The organism externally contracts six exogenous pathogenic factors,and internally corresponds to five zang-organs and six fu-organs,then the disease must be different from cold and heat.Syndrome differentiation of deficiency and excess can understand exuberance and decline of pathogenic factors and healthy qi,excess syndrome should be applied with purging,while deficiency syndrome should be applied with tonifying,which can provide theoretical basis for treatment.One typical case is attached for verification.
