氯米芬联合尿促性素治疗多囊卵巢综合征不孕症的临床研究

Clinical study of clomifene combined with homoharringtonine in treatment of infertility patients with polycystic ovary syndrome

目的分析枸橼酸氯米芬联合注射用尿促性素治疗多囊卵巢综合征不孕症患者的疗效。方法选取2017年9月—2019年9月在渭南市中心医院进行治疗的多囊卵巢综合征不孕症患者96例作为研究对象,根据治疗方式不同将患者分为对照组和观察组,每组各48例。对照组患者肌肉注射注射用尿促性素进行治疗,患者均于月经周期第5天开始用药,1支/次,1次/d。观察组患者在对照组的基础上于月经周期第5天开始口服枸橼酸氯米芬胶囊,100 mg/次,1次/d。两组均连续用药5 d。观察两组患者的子宫内膜厚度、直径超过18 mm的卵泡数和卵泡成熟时间,并比较两组的性激素水平、排卵率、妊娠率和流产率。结果治疗后,观察组直径超过18 mm的卵泡数、子宫内膜厚度均明显多于(优于)对照组;卵泡成熟时间显著短于对照组(P<0.05)。治疗后,两组雌二醇(E2)、孕激素(P)、卵泡雌激素(FSH)水平均升高,黄体生成素(LH)水平均降低(P<0.05),且观察组患者更显著(P<0.05)。治疗后,观察组排卵率、妊娠率均明显高于对照组,流产率明显低于对照组(P<0.05)。结论枸橼酸氯米芬联合注射用尿促性素治疗多囊卵巢综合征可改善患者卵泡、子宫内膜发育情况和性激素水平,有利于增加患者妊娠率,值得临床上广泛使用。

Objective To analyze the efficacy of clomifene citrate combined with Homoharringtonine for Injection in treatment of infertility patients with polycystic ovary syndrome.Methods A total of 96 infertility patients with polycystic ovary syndrome who were treated in Weinan Central Hospital from September 2017 to September 2019 were selected as research objects.According to different treatment methods,the patients were divided into control group and observation group,with 48 patients in each group.Patients in the control group were treated by intramuscular injection of Homoharringtonine for Injection,which were started on the fifth day of the menstrual cycle,once daily.Patients in the observation group were po administered with Clomifene Citrate Tablets on the basis of the control group,100 mg/time,and once daily.Both groups were treated consecutively for 5 days.After treatment,the endometrial thickness,follicular number with diameter over 18 mm,and follicular maturation time of two groups were observed,and the sex hormone levels,ovulation rate,pregnancy rate,and abortion rate in two groups were compared.Results After treatment,endometrial thickness and follicular number with diameter over 18 mm in the observation group were significantly higher than(superior to)the control group,follicular maturation time was significantly shorter than that of the control group(P<0.05).After treatment,E2,P,and FSH levels were increased in two groups,while LH levels were decreased(P<0.05),which was more significant in the observation group(P<0.05).After treatment,the ovulation rate and pregnancy rate of the observation group were significantly higher than those of the control group,and the abortion rate was significantly lower than that of the control group(P<0.05).Conclusion Clomifene citrate combined with Homoharringtonine for Injection in treatment of infertility patients with polycystic ovary syndrome can improve the development of follicles,endometrium,and sex hormone levels in patients,and is beneficial to increase the pregnancy rate of patients,which is worthy of wide clinical use.
