血小板微粒水平与急性脑梗死临床相关性研究 被引量:1

摘要 目的分析血小板微粒(platelet microparticle,PMP)水平与急性脑梗死(acute cerebral infarction,ACI)的相关性。方法随机招募2015年10月至2017年12月就诊于本院神经内科的ACI患者共127例,归为ACI组,同期招募于辽宁省人民医院体检的健康志愿者127人归为健康对照组(HC组)。两组患者入组后均收集临床基线资料,根据量表计算ACI患者入院NIHSS评分,抽取静脉血进行PMP等实验室指标检测,比较两组患者临床资料的差异,应用多因素Logistic回归分析PMP与ACI的相关性,通过受试者工作特征(ROC)曲线分析PMP诊断ACI的临床价值。结果ACI组PMP、MPV、Hcy及LDL-C水平显著高于HC组;NIHSS评分越高,PMP水平越高,两者呈正相关关系(r=0.421,P<0.01);多因素Logistic回归分析发现PMP是ACI的危险因素,其优势比(OR)(95%CI)为3.27(2.51~7.04);PMP诊断ACI的价值曲线下面积(AUC)(95%CI)为0.85(0.80~0.90);差异均具有统计学意义。结论PMP是ACI发病的独立危险因素,同时是潜在的临床诊断标志物。 Objective To analyze the relationship between level of platelet microparticles(PMP)and acute cerebral in-farction(ACI).Methods A total of 127 ACI patients were collected in this study in the Department of Neurology from October 2015 to December 2017 as ACI group,and 127 healthy volunteers were recruited as healthy control group(HC group)at the same time.The clinical baseline data were collected from them,the NIHSS scores of patients calculated according to the scale and venous blood was drawn for laboratory indicators such as PMP etc,to compared the clinical data between two groups,and to analyzed the relationship between PMP and ACI by using multivariale Logistic regres-sion,the value of PMP for diagnosis of ACI was analyzed by receiver operating characteristic(ROC)curve.Results The levels of PMP,MPV,Hcy and LDL-C in ACI group were signfficantly higher than those of HC group;Higher NIHSS score and higher level of PMP,it had a positive corelation with them(r=0.421,P<0.01);Multivariate logistic regression analysis found that PMP was a risk factor for ACI,and its odds ratio(OR)(95%CI)was 3.27(2.51~7.04);The area un-der ROC curve(AUC)(95%CI)of PMP for diagnosis ACI was 0.85(0.80~0.90);all of P value were less than 0.05.Conclusion PMP is an independent risk factor for ACI and severs as a potential clinical diagnostic marker.
