miR-21和miR-146a在癫痫患者中的表达及临床意义

The expression and clinical significance of serum miR-21 and miR-146a in patients with epilepsy

目的分析微小RNA-21(miR-21)和miR-146a在癫痫患者血清中的表达及临床意义。方法选择2017年4月—2019年5月成都中医药大学附属第五人民医院/成都市第五人民医院神经内科诊治的癫痫患者99例作为癫痫组,选择同期在医院进行体检的健康者99例作为健康对照组。比较2组血清miR-21和miR-146a表达水平,血清肿瘤坏死因子-α(TNF-α)和白介素-6(IL-6)水平,分析血清miR-21和miR-146a表达水平与NHS3评分、血清TNF-α、IL-6水平的相关性,采用受试者工作特征(ROC)曲线分析血清miR-21和miR-146a对癫痫的诊断价值。结果与健康对照组比较,癫痫组血清miR-21、miR-146a、TNF-α、IL-6水平均明显升高(t=17.272、18.781、27.119、26.240,P均=0.000)。血清miR-21和miR-146a表达水平与NHS3评分、血清TNF-α、IL-6均呈正相关(r=0.832,0.624,0.619,0.616,0.529,0.352,P=0.000)。血清miR-21、miR-146a及二者联合诊断癫痫的AUC分别为0.760,0.793,0.884。结论癫痫患者血清miR-21和miR-146a表达水平升高,与炎性因子和NHS3评分有关,检测miR-21和miR-146a在癫痫病情评估和诊断中具有一定的辅助价值。

Objective To analyze the expression and clinical significance of microRNA-21(miR-21)and miR-146a in the serum of patients with epilepsy.Methods From April 2017 to May 2019,99 patients with epilepsy diagnosed and treated by the Department of Neurology of Chengdu Fifth People's Hospital/Chengdu Fifth People's Hospital were selected as the epilepsy group,and 99 healthy patients who underwent physical examination in the hospital during the same period were selected as the epilepsy group.Healthy control group.The serum levels of miR-21 and miR-146a,serum tumor necrosis factorα(TNF-α)and interleukin 6(IL-6)levels were compared between the two groups,and the correlation between serum miR-21 and miR-146a expression levels and NHS3 scores,serum TNF-αand IL-6 levels were analyzed.Subjects were used Operating characteristic(ROC)curve analysis of the diagnostic value of serum miR-21 and miR-146a for epilepsy.Results Compared with the healthy control group,serum miR-21,miR-146a,TNF-α,and IL-6 levels in the epilepsy group were significantly increased(t=17.272,18.781,27.119,26.240,all P=0.000).The expression levels of serum miR-21 and miR-146a were positively correlated with NHS3 score,serum TNF-α,IL-6(r=0.832,0.624,0.619,0.616,0.529,0.352,P=0.000).The AUC of serum miR-21,miR-146a and their combined diagnosis of epilepsy were 0.760,0.793,0.884,respectively.Conclusion The elevated serum levels of miR-21 and miR-146a in patients with epilepsy are related to inflammatory factors and NHS3 score.The detection of miR-21 and miR-146a has certain auxiliary value in the evaluation and diagnosis of epilepsy.
