磁珠耳穴贴压对乳腺癌患者创伤后应激障碍的干预效果评估

Evaluation of intervention effect of magnetic beads auricular point plaster sticking on post-traumatic stress disorder in patients with breast cancer

目的探究磁珠耳穴贴压对乳腺癌患者创伤后应激障碍(PTSD)的干预效果。方法选取2018年9月至2019年12月就诊于中国中医科学院广安门医院的乳腺癌PTSD患者88例。采用随机数字表法分为观察组和对照组,各44例。2组患者均接受西医抗肿瘤规范治疗及中医综合治疗,观察组患者在此基础上行磁珠耳穴贴压治疗,治疗周期均为12周。观察分析干预前后2组患者的PTSD自评量表(PCL-C)、焦虑自评量表(SAS)、抑郁自评量表(SDS)、汉密尔顿抑郁量表(HAMD)、症状自评量表(SCL-90)评分及失眠严重指数(ISI)的变化。结果干预后2组PCL-C、SAS、SDS、HAMD、SCL-90评分及ISI水平均较治疗前降低,且观察组HAMD、SCL-90评分及ISI低于对照组[(15±4)分比(17±5)分、(150±19)分比(161±26)分、(5.6±2.1)比(9.1±3.3)],差异均有统计学意义(均P<0.05)。干预后2组PCL-C、SAS、SDS评分差异均无统计学意义(均P>0.05)。结论在中医理论指导下的中医综合治疗可改善乳腺癌患者的PTSD、焦虑抑郁等不良情绪,同时对改善睡眠状况有较为显著的效果。采用磁珠耳穴贴压对治疗乳腺癌PTSD有协同增效作用。

Objective To explore the intervention effect of magnetic beads auricular point plaster sticking on post-traumatic stress disorder(PTSD)in patients with breast cancer.Methods From September 2018 to December 2019,88 breast cancer patients with PTSD were selected from Guang’anmen Hospital,China Academy of Chinese Medicine Sciences.They were randomly divided into observation group and control group,with 44 cases in each group.Both groups received anti-tumor standard treatment and integrated Chinese medicine treatment.Patients in the observation group were additionally treated with auricular points magnetic beads plaster sticking on this basis,all patients were treated for 12 weeks.The scores of PTSD checklist-civilian version(PCL-C),self-rating anxiety scale(SAS),self-rating depression scale(SDS),Hamilton Depression Scale(HAMD),symptom checklist-90(SCL-90)scores and the insomnia severity index(ISI)in the two groups were observed and analyzed before and after intervention.Results After intervention,PCL-C,SAS,SDS,HAMD,SCL-90 scores and ISI in the two groups were lower than those before treatment,and the scores of HAMD,SCL-90 and ISI in the observation group were lower than those in the control group[(15±4)vs(17±5),(150±19)vs(161±26),(5.6±2.1)vs(9.1±3.3)](all P<0.05).There were no significant differences in PCL-C,SAS and SDS scores in the two groups after intervention(all P>0.05).Conclusions The PTSD,anxiety,depression and sleep status of breast cancer patients can be improved by the integration treatment with traditional Chinese medicine guidance.The use of auricular points magnetic beads plaster sticking may has a synergistic effect on the treatment of breast cancer patients with PTSD.
