宫颈癌患者配偶照顾疲劳及影响因素分析

Analysis on Fatigue in Spouses of Patients with Cervical Cancer and Its Influencing Factors

目的了解宫颈癌患者配偶的照顾疲劳感现状,并分析其影响因素。方法选取2018年3月—2019年1月在河南省人民医院放疗科行宫颈癌根治术后放疗的患者配偶240名为调查对象。采用一般资料调查表、疲劳量表、照顾者压力量表、简易应对方式量表对其进行调查。结果宫颈癌患者配偶疲劳量表总分为(8.92±1.64)分,85.00%的宫颈癌患者配偶处于中、重度疲劳;宫颈癌患者配偶在躯体疲劳感、心理疲劳感及疲劳总分上均高于癌症患者配偶(P<0.05),宫颈癌患者配偶的照顾压力3个维度(身体社交压力、心理压力、工作经济压力)、消极应对与疲劳感呈正相关(P<0.05),积极应对与疲劳感呈负相关(P<0.05)。多元线性回归分析显示,照顾压力、照顾时间、应对方式是宫颈癌患者配偶疲劳感的影响因素,可解释60.40%的变异量。结论宫颈癌患者配偶照顾疲劳发生率高,临床医护人员应针对配偶照顾疲劳感的影响因素实施有针对性的干预措施,从而减轻宫颈癌患者配偶照顾疲劳,进而提高其对患者的照顾质量,促进患者疾病的转归。

Objective To understand the fatigue in the spouses of patients with cervical cancer and analyze the influencing factors.Methods A total of 240 spouses of patients who underwent radiotherapy after radical hysterectomy in the Department of Radiotherapy,Henan Provincial People′s Hospital from March 2018 to January 2019 were selected as the subjects.They were investigated by using the General Information Questionnaire,Fatigue Scale,Caregiver Strain Index,and Simplified Coping Style Questionnaire.Results For the spouses of patients with cervical cancer,the total score of Fatigue Scale was(8.92±1.64)points,85.00%of them were in moderate and severe fatigue.For the spouses of patients with cervical cancer,the score of phy-sical fatigue and psychological fatigue and the total score of fatigue were higher than those of the spouses of patients with cancer(P<0.05).The 3 stress dimensions(physical and social stress,mental stress,work and economic stress)and negative coping style were positively correlated with the fatigue in the spouses of patients with cervical cancer(P<0.05),and positive coping style was negatively correlated with fatigue(P<0.05).Multiple linear regression analysis showed that the fatigue in the spouses of cervical cancer patients was affected by caregiving stress,caregiving time and coping style,which could explain 60.40%of the variance.Conclusion The incidence of fatigue in spouses of patients with cervical cancer is high.Clinical medical staff should take targeted measures according to the influencing factors of spouse fatigue,so as to reduce the fatigue in spouses of patients with cervical cancer,improve their quality of care and promote the outcome of patients′diseases.
