以人文关怀为核心的综合疼痛管理在骨科手术患者中的应用分析

Application analysis of comprehensive pain management with humanistic care in patients undergoing orthopedic surgery

目的探讨以人文关怀为核心的综合疼痛管理在骨科手术患者中的应用分析。方法以我院2017年9月~2019年5月收治的218例骨科手术患者作为研究对象,其中采用常规护理的129例患者为对照组,89例采用常规护理联合综合疼痛管理的患者为观察组,对护理人员疼痛管理模式改良前后的认知水平以及患者的疼痛管理情况进行评估和比较。结果护理人员培训后其疼痛相关知识、疼痛评估、疼痛干预、疼痛教育与合作等疼痛管理及相关人文关怀理论认知正确率均较培训前提高,差异具有统计学意义(P<0.05);术后72h内观察组患者的自觉疼痛评分均低于对照组,在镇痛需求上观察组患者告知疼痛26例,计划外药物镇痛22例,相比对照组发生55例告知疼痛和计划外药物镇痛发生次数减少,差异均具有统计学意义(P<0.05)。结论提高医护人员人文关怀认知水平,让患者参与疼痛管理的同时提高疼痛护理的质量对于改善患者术后疼痛程度有显著的效果。

Objective To explore the application of comprehensive pain management with humanistic care in patients undergoing orthopedic surgery.Methods To select 218 cases of orthopedic surgery patients from September 2017 to May 2019 as the research objects,129 cases of patients with conventional nursing were control group,89 cases of patients with normal nursing combined comprehensive pain management were observation group.Then compared and analyzed the cognitive level,and the situation of patient′s pain management before and after the improvement of nurses′ pain management model.Results After the training,the correct cognitive rates of pain management and related humanistic care theories such as pain related knowledge,pain assessment,pain intervention,pain education and cooperation were all improved,the difference had statistically significant(P<0.05).In terms of analgesia demand,26 patients in the observation group were informed of pain,and 22 patients in the observation group had unplanned analgesia.Compared with the control group which has 55 patients with pain informed and unplanned analgesia,the observation group was decreased significantly(P<0.05).Conclusion Improving the cognition level of humanistic care in medical staff,involving patients in pain management and improving the quality of pain care had a significant effect on improving the degree of postoperative pain of patients.
