基于前馈控制理念的两种ICU访视方式在择期手术患者中的研究

Study on the two ICU visit methods based on the idea of feedforward control in patients undergoing electivesurgery

目的探讨基于前馈控制理念的两种ICU访视方式在择期手术患者中的应用效果。方法选取2018年9月—2019年3月预约入住ICU的93例择期手术患者为研究对象,随机分为试验组(44例)和对照组(49例),对照组由ICU护士结合自制图谱访视,试验组采用自制视频访视方式,从访视前后焦虑自评量表(SAS)评分,ICU环境压力源量表(ICUESS)评分,患者满意度,ICU住院时间、机械通气时间、丙泊酚用量、保护性约束时间等方面对2组进行比较。结果试验组患者满意度93.18%,明显高于对照组;试验组访视后焦虑自评量表评分为(47.02±4.50)分,ICU环境压力源量表评分为(29.29±15.71)分,ICU住院时间为(27.52±13.44)h、机械通气时间为(15.98±4.73)h、丙泊酚用量为(1.40±0.72)g、保护性约束时间为(24.40±12.60)h,均明显低于对照组(P<0.05)。结论基于前馈控制理念的ICU视频访视方式,能有效降低择期手术患者的焦虑程度,减少ICU环境压力来源,提高患者对ICU护理服务满意度,以及缩短ICU住院时间、机械通气时间、丙泊酚用量、保护性约束时间。

Objective To explore the effect of two ICU visit methods based on the idea of feedforward control in patients undergoing elective surgery.Methods 93 patients with elective surgery who were admitted to ICU by appointment from September 2018 to March 2019 were selected as study subjects and randomly divided into experimental group(44 cases)and control group(49 cases).The control group was visited by ICU nurses in combination with self-made atlas,the experimental group adopted self-made video visiting method.The anxiety self-rating scale(SAS)score,ICU environmental Stress Scale(ICUESS)score,patient satisfaction,ICU stay,mechanical ventilation time,propofol dosage,and protective restraint time were compared between the two groups before and after the visit.Results The satisfaction of patients in the experimental group was 93.18%,which was significantly higher than that in the control group;the score of self rating Anxiety Scale(SAS)was 47.02±4.50,the score of ICU environmental stressor scale was 29.29±15.71,ICU hospitalization time was 27.52±13.44 hours,mechanical ventilation time was 15.98±4.73 hours,propofol dosage was 1.40±0.72 g.The protective restraint time was 24.40±12.60 hours,which was significantly lower than that of the control group(P<0.05).Conclusion Video visit in ICU based on the concept of feedforward control can effectively reduce the anxiety,reduce the source of environmental pressure in ICU,improve patients'satisfaction with ICU nursing services,and shorten the length of stay in ICU,mechanical ventilation time,propofol dosage,and protective restraint time.
