上海市青浦区养老机构感染控制综合干预效果分析

Analysis of effect of comprehensive intervention on infection control in elderly care institutions in Qingpu of Shanghai

目的探讨养老机构感染控制综合干预效果。方法2015年10月-2018年10月随机抽取青浦区规模和卫生条件相近且配合程度高的6所养老机构,采用调查问卷方法对工作人员进行卫生知识调查,收集老年人感染性疾病资料和监测消毒效果。结果养老机构工作人员个人防护使用知识条目的知晓率最高,为80.00%;消毒隔离知识条目的知晓率最低,为24.55%。监测样品消毒合格率为72.76%,其中抹布的合格率最低,其次是餐饮具。共调查养老机构老年人337人,年内报告新发感染性疾病134人次,发病率为39.76%。干预后,干预组工作人员卫生知识知晓率和监测样品消毒合格率高于对照组(P<0.01);干预组老年人感染性疾病发病率显著低于干预前发病率(P<0.01)。结论综合干预可以降低老年人感染性疾病发病率,应提高工作人员相关卫生知识知晓率及各环节消毒合格率。

Objective To explore the effect of comprehensive intervention on infection control in elderly care institutions.Methods From October 2015 to October 2018,6 elderly care institutions with similar scale and sanitary condition and high degree of coordination were randomly selected to conduct hygiene knowledge surveys on staff,collect information on infectious diseases of the elderly and monitor disinfection effects.Results The awareness rate of knowledge of using personal protective equipment among the staff of elderly care institutions was the highest at 80%,while the awareness rate of disinfection and isolation knowledge was the lowest at 24.55%.The qualified rate of disinfection of monitored samples was 72.76%,among which the qualified rate of wipes was the lowest,followed by tableware.A total of 337 elderly people in elderly care institutions were surveyed,and 134 new infectious diseases were reported in the last year,with an incidence rate of 39.76%.After the intervention,the awareness rate of hygiene knowledge of staff and the qualified rate of disinfection of monitored samples in the intervention group were significantly higher than those in the control group(P<0.01);the incidence of infectious disease in the elderly of the intervention group was significantly lower than that before intervention(P<0.01).Conclusion Comprehensive intervention on infection control can reduce the incidence of infectious disease in the elderly.The awareness rate of relevant hygiene knowledge of the staff and the qualified rate of disinfection should be improved.
