随着人们对医疗服务需求的增加,护理的重心已经转移向社区及家庭,并向疾病预防转变,延续护理已成为整体护理的一部分。介绍国内对于冠心病患者延续护理的现状,旨在为我国冠心病患者延续护理工作提供参考和借鉴,探索适宜的护理模式,促进我国冠心病患者延续护理的发展,提高患者生活质量。

With the increase of people’s demand for medical services,the focus of care has shifted to the community and the family,and to disease prevention,continuation of care has become a part of the overall care.This paper introduces the status quo of continuous nursing care for patients with coronary heart disease in China,aiming at providing reference and reference for the continuous nursing work of patients with coronary heart disease in China,exploring the appropriate nursing model,promoting the development of continuous nursing care for patients with coronary heart disease in China,and improving the quality of the life of patients.
