利拉鲁肽与西格列汀联合二甲双胍对2型糖尿病患者胰岛素抵抗、血管内皮功能的影响

Effects of liraglutide and sitagliptin combined with metformin on insulin resistance and vascular endothelial function in patients with type 2 diabetes

目的探讨利拉鲁肽与西格列汀联合二甲双胍对2型糖尿病患者胰岛素抵抗、血管内皮功能的影响。方法选取2018年1月至2020年1月收治的92例2型糖尿病患者作为研究对象,采用抽签法将其随机分为A组(46例,利拉鲁肽+二甲双胍)和B组(46例,西格列汀+二甲双胍)。比较两组治疗前、后的血糖指标、BMI、胰岛素抵抗指数、胰岛β细胞功能指数及血管内皮功能指标。结果治疗后,A组的FPG、HbA1c及BMI水平均明显低于B组(P<0.05)。治疗后,A组的胰岛素抵抗指数明显低于B组,胰岛素β细胞功能指数明显高于B组(P<0.05)。治疗后,A组的脂联素、NO水平均明显高于B组,ET水平明显低于B组(P<0.05)。结论利拉鲁肽与西格列汀联合二甲双胍治疗2型糖尿病患者均能取得较好的治疗效果,且利拉鲁肽对血糖、BMI、胰岛素抵抗及血管内皮功能的改善效果优于西格列汀,值得临床推广和应用。

Objective To explore the effects of liraglutide and sitagliptin combined with metformin on insulin resistance and vascular endothelial function in patients with type 2 diabetes.Methods A total of 92 patients with type 2 diabetes admitted from January 2018 to January 2020 were selected as the research objects and randomly divided into group A(46 cases,liraglutide+metformin)and group B(46 cases,sitagliptin+metformin)by drawing lots.The blood glucose index,BMI,insulin resistance index insulinβ-cell function index and vascular endothelial function of the two groups before and after treatment were compared.Results After treatment,FPG,HbA1c and BMI levels in the group A were significantly lower than those in the group B(P<0.05).After treatment,the insulin resistance index in the group A was significantly lower than that in the group B,and the insulinβ-cell function index was significantly higher than that in the group B(P<0.05).After treatment,the levels of adiponectin and NO in the group A were significantly higher than those in the group B,and the ET level was significantly lower than that in the group B(P<0.05).Conclusion Liraglutide and sitagliptin combined with metformin can both achieve good therapeutic effects in the treatment of type 2 diabetes patients,but the effect of liraglutide on blood sugar,BMI,insulin resistance and vascular endothelial function is better than sitagliptin,which is worthy of clinical promotion and application.
