膝关节置换术治疗膝关节退行性病变的临床效果

Clinical effect of knee arthroplasty in the treatment of retrogressive knee-joint lesions

目的探讨膝关节置换术治疗膝关节退行性病变的临床效果。方法将122例膝关节退行性病变患者按照治疗方式分为对照组(61例,关节镜下清理术)与观察组(61例,膝关节置换术)。比较两组的临床效果。结果术后3、12个月,两组的疼痛、关节功能、关节活动度、肌力、屈曲畸形、关节稳定性评分及总分均高于术前,且观察组高于对照组(P<0.05)。术后12个月,两组的膝关节活动度及最大屈曲度均大于术前,且观察组大于对照组(P<0.05)。术后,两组的SF-36评分均升高,VAS评分均降低,等长收缩最大肌力均增大,且观察组优于对照组(P<0.05);术后,观察组的随意运动肌力小于对照组(P<0.05)。术后12个月,两组的FJS评分均高于术后6个月,且观察组高于对照组(P<0.05)。结论膝关节置换术能有效改善膝关节退行性病变患者的膝关节功能,提高下肢运动能力,缓解疼痛。

Objective To explore the clinical effect of knee arthroplasty in the treatment of retrogressive knee-joint lesions.Methods A total of 122 patients with retrogressive knee-joint lesions were divided into control group(61 cases,arthroscopic debridement)and observation group(61 cases,knee arthroplasty)by the treatment methods.The clinical effects of the two groups were compared.Results At 3 and 12 months after operation,the scores of pain,joint function,range of motion of joint,muscle strength,flexion deformity,joint stability scores and total score in the two groups were higher than those before operation,and those in the observation group were higher than the control group(P<0.05).At 12 months after operation,the range of motion and maximum flexion of the knee joint in the two groups were greater than those before operation,and those in the observation group were greater than the control group(P<0.05).After operation,SF-36 score in the two groups increased,VAS score decreased,isometric contraction maximum muscle strength enlarged,and those in the observation group were better than the control group(P<0.05);after operation,the random exercise muscle strength in the observation group was smaller than that in the control group(P<0.05).At 12 months after operation,the FJS score in the two groups were higher than that at 6 months after operation,and that in the observation group was higher than the control group(P<0.05).Conclusion Knee arthroplasty can effectively improve the knee-joint function in patients with retrogressive knee-joint lesions,and the motor ability of lower limbs and relieve pain.
