乳岩宁方对绝经后乳腺癌内分泌治疗患者生活质量及免疫功能的影响

Effect of Ruyanning Formula on Quality of Life and Immune Function of Postmenopausal Breast Cancer Patients Undergoing Endocrine Therapy

目的观察乳岩宁方对绝经后乳腺癌内分泌治疗患者生活质量及免疫功能的影响。方法采用随机数字表法将134例正在接受内分泌治疗且具有类更年期综合征的绝经后乳腺癌患者随机分为对照组和观察组各67例。对照组采用来曲唑治疗,每日1次口服,观察组在对照组基础上加用乳岩宁方早晚各1次口服,两组疗程为2个月,分析两组改良Kupperman症状积分、生活质量、中医症状积分、中医证候疗效、血脂变化、免疫功能状态及不良反应。结果实际完成研究121例,对照组60例、观察组61例。治疗后观察组改良Kupperman症状积分较之前显著下降(P<0.05),改良Kupperman症状积分观察组明显低于对照组(P<0.05);治疗后观察组在生理状况、社会/家庭状况、情感状况、功能状况、附加关注和总分等生活质量项目与同组治疗前及对照组治疗后比较均有改善(P<0.05);中医症状积分比较,治疗后对照组与治疗前比较,潮热盗汗、五心烦热较前加重(P<0.05);治疗后观察组在潮热盗汗、情绪抑郁、烦躁易怒、五心烦热、乏力倦怠、头晕耳鸣、总分等项目中较治疗前及对照组治疗后比较均有改善(P<0.05);两组治疗后总有效率观察组明显高于对照组(P<0.01);观察组治疗后免疫指标CD3、CD4、CD4/CD8较同组治疗前及对照组治疗后水平有改善(P<0.05);治疗后甘油三酯水平观察组明显低于对照组(P<0.05);两组患者经过内分泌治疗前后对照无明显WBC、Hb、PLT、ALT、AST、Cr、BUN差异(P>0.05)。结论乳岩宁方对改善绝经后乳腺癌患者内分泌治疗生活质量及免疫功能有较好效果。

Objective To observe the effect of Ruyanning Formula on quality of life and immune function of breast cancer patients undergoing endocrine therapy. Methods Random number table method was used to randomly divide134 postmenopausal breast cancer patients who had endocrine therapy and menopausal syndrome into a control group and an observation group with 67 cases in each group. The control group was treated with letrozole once a day, while the observation group was treated with Ruyanning Formula once in the morning and once in the evening in addition to the treatment of the control group. The course of treatment in the two groups was 2 months. The modified Kupperman symptom score, quality of life, TCM symptom scores, curative effect of traditional Chinese medicine syndrome, blood lipid changes, immune function status, and adverse reactions of the two groups were analyzed. Results The experiment was completed in 121 cases, 60 cases in the control group and 61 cases in the observation group. After treatment, the modified Kupperman symptom score in the observation group decreased significantly(P < 0.05), and the modified Kupperman symptom score in the observation group was significantly lower than that in the control group(P < 0.05).After treatment, the physiological status, social/family status, emotional status, functional status, additional attention and total score of life quality items in the observation group were improved compared with those before treatment in the same group and after treatment in the control group(P < 0.05);Comparison of TCM symptom scores: comparing the symptoms of cases in the control group before and after treatment, tidal fever and night sweating, feverish sensation over the five centers were worse than those before treatment(P < 0.05);after treatment, compared with the control group before and after treatment, the symptoms of cases in the observation group were improved in tidal fever and night sweating,depression, anxiety and irritability, feverish sensation over the five centers, weakness and fatigue, dizziness and tinnitus,total score and other items(P < 0.05);the total effective rates of the observation group were significantly higher than those of the control group(P < 0.01). CD3, CD4, CD4/CD8 and other indexes in the observation group after treatment were better than those before treatment(P < 0.05). Moreover, CD3, CD4, CD4/CD8 and other indexes in the observation group were better than those in the control group after treatment(P < 0.05). After treatment, the triglycerol level in the observation group was lower than that in the control group(P < 0.05). There was no significant difference in WBC, Hb,PLT, ALT, AST, Cr and BUN between the two groups before and after endocrine therapy(P > 0.05). Conclusion Ruyanning Formula has a good effect on improving the quality of life and immune function of postmenopausal breast cancer patients after endocrine therapy.
