5种指标评定中国老年人中心性肥胖的可行性

摘要 选取1000名60~75岁老年人为研究对象,获取其腰围(WC)、腰臀比(WHR)、腰身比(WSR)、体质指数(BMI)、人体肥胖指数(BAI)及相对脂肪含量(RFM)等指标,运用SPEARMAN法进行WC同其他肥胖指标的相关性分析。以WC为基准,运用ROC曲线描述各指标评价中心性肥胖的价值并估算各指标评定中心性肥胖的最佳切点。结果显示,5种肥胖指标同WC均呈正相关,其中WSR和RFM的相关度最高,均呈极强相关(r>0.9)。各指标的ROC曲线面积均大于0.5,表明在预测中心性肥胖中均有价值;其中RFM和WSR面积最大,男、女均大于0.9,有较高准确性。WHR、WSR、BAI、BMI和RFM的ROC最佳切点值,男性老年人依次为0.92、0.56、28.54、24.31和27.19;女性老年人依次为0.94、0.56、31.57、23.28和39.90。因此,以WC为标准,WHR、WSR、BAI、BMI、RFM均可用于评定中国老年人中心性肥胖,其中WSR和RFM参考价值最高。 To evaluate the central obesity of the elderly in China,the waist circumference(WC),waist-hip ratio(WHR),waist-body ratio(WSR),body mass index(BMI),body fat index(BAI)and relative fat content(RFM)of 1000 elderly people aged from 60 to 75 were measured.By using Spearman methods,the correlation between WC and other obesity indexes was analyzed.Furthermore,the value and optimal tangency point of each index in evaluating central obesity were described with ROC curve.It was found that the five obesity indexes were all positively correlated with WC,in which WSR and RFM had the highest correlation(r>0.9).The ROC curve area of greater than 0.5 of each index showed that they were all valuable in predicting central obesity,among which the areas of RFM and WSR were the largest(>0.9).The optimal ROC tangency point value of WHR,WSR,BAI,BMI and RFM was 0.92,0.56,28.54,24.31 and 27.19,respectively for the male,while was 0.94,0.56,31.57,23.28 and 39.90 for the male,respectively.It is concluded that,with WC as the standard,WHR,WSR,BAI,BMI and RFM can be used to evaluate the central obesity of the elderly in China,in which WSR and RFM have the highest reference value.
