冲压法上颌窦底提升术后种植体周围骨改建及影响因素分析

Analysis of bone remodeling around implants and the influencing factors after osteotome sinus floor elevation

目的:分析牙槽嵴严重萎缩患者在冲压法上颌窦底提升术后种植体周围骨改建情况及其影响因素。方法:纳入患者41例,根据剩余牙槽骨高度(RBH)分为A组(2<RBH≤4 mm,20例)和B组(4<RBH≤6 mm,21例)。术后1年和3年,用影像学测量技术比较2组种植体周围的骨改建情况,并建立多元线性回归模型。结果:术后1、3年时,A组的垂直骨增量(VBG)均高于B组(P<0.05);术后3年时VBG与种植体长度(IL)呈正相关(r=0.284,P<0.05),与RBH呈负相关(r=-0.409,P<0.05);方程:术后3年VBG=0.836 IL-1.074 RBH-0.424。结论:RBH和IL是VBG的重要影响因素。

Objective:To analyze the bone remodeling around the implants and its influencing factors in patients with severe alveolar bone atrophy after osteotome sinus floor elevation.Methods:41 patients underwent osteotome sinus floor elevation were divided into 2 groups based on the residual bone height(RBH):2<RBH≤4 mm(group A,n=20)and 4<RBH≤6 mm(group B,n=21).1 and 3 years after operation the bone remodeling around the implants was analyzed with morphological measurements and the multiple linear regression equation was established.Results:The Vertical bone gain(VBG)in group A was higher than that in group B 3 years after surgery(P<0.05);VBG has a positive correlation with IL(r=0.284,P<0.05),and a negative correlation with RBH(r=-0.409,P<0.05);an equation was constructed:VBG=0.836 IL-1.074 RBH-0.424.Conclusion:RBH and IL are the most signifiacnt paremeters influencing VBG.
