超声引导下股神经阻滞用于膝关节置换术后镇痛疗效分析 被引量:7

摘要 目的探讨超声引导下股神经阻滞对膝关节置换患者术后康复效果的影响。方法选取择期行单侧膝关节置换手术患者60例为观察对象,采用随机数字表法,根据不同的术后镇痛方式分为实验组(F组)、对照组(C组),每组各30例。F组采用股神经阻滞+静脉镇痛+关节囊注射镇痛方式,C组采用静脉镇痛+关节囊注射镇痛方式。记录两组患者在术后4 h、8h、12h、24 h、48 h静息时和活动时NRS评分,记录两组患者静脉镇痛泵按压次数及药物使用总量,记录两组术后4h、8 h、12 h、24 h、48 h术侧股四头肌肌力分级及不良反应发生情况。结果F组各时间点NRS评分均低于C组(P<0.05),F组患者静脉镇痛泵按压次数及药物使用总量均低于C组(P<0.05),F组患者的恶心呕吐、眩晕发生率和嗜睡发生率明显低于C组(P<0.05)。然而F组各时间点股四头肌肌力分级均低于C组。结论超声引导下股神经阻滞可有效降低患者术后NRS评分,不良反应少,更有利于患者术后恢复。 Objective To investigate the effect of ultrasound-guided femoral nerve block on the rehabilitation of patients after knee replacement.Methods Sixty patients underwent elective unilateral knee replacement surgery were selected as observation objects.Using random number table method,they were divided into experimental group(group F)and control group(group C)according to different postoperative analgesia methods.30 cases in each.The experimental group used femoral nerve block+intravenous analgesia+joint capsule injection analgesia,and the control group used intravenous analgesia+joint capsule injection analgesia.The NR.S scores of the two groups of patients were recorded at rest and during activity at 4 h,8 h,12 h,24 h,48 h after surgery,the number of intravenous analgesia pump compressions and the total amount of drug use were recorded in the two groups,the 4 h,8 h,and 12 h,24 h,48 h quadriceps muscle strength classification and adverse reactions were recorded on the surgical side.Results The NRS scores of group F were lower than those of group C(P<0.05)at all time points.The number of intravenous analgesia pump compressions and the total amount of drugs used in group F were lower than those of group C(P<0.05).Patients in group F had nausea and vomiting The incidence of vertigo and drowsiness were significantly lower than those of the control group(P<0.05).However,the quadriceps muscle strength grades of group F were lower than those of group C at all time points.Conclusion Ultrasound-guided femoral nerve block can effectively reduce the postoperative NRS score of patients with fewer adverse reactions,and is more conducive to postoperative recovery.
