国际疾病诊断相关分组(DRG)研究:进展与趋势——基于Citespace和VOSviewer的文献计量分析

International Diagnosis Related Groups (DRG) research:progress and trends——Bibliometric analysis based on Citespace and VOSviewer

目的对国际DRG研究领域研究进展与趋势进行分析并总结,为我国DRG研究提供新的视角和参考。方法利用Citespace知识图谱分析工具和VOSviewer可视化分析软件,对1981年至2019年Web of Science数据库核心合集收录的2473篇DRG领域研究文献的发文数量、高发文机构分布、高发文期刊来源和国家/地区分布、高发文国家/地区科研合作、高发文作者科研合作、主要研究热点及变化趋势等进行计量分析。结果与结论研究结果显示,1981~2019年国际DRG研究文献数量总体呈波动上升趋势,DRG研究在世界范围内具有持续热度。美国在DRG领域的研究始终处于国际前沿。DRG的研究热点主要体现在三个方面:a.基于疾病危重度的病人分组工具的开发和测评研究;b.实施以DRG为基础的预付制付费方式对医疗机构绩效的影响研究;c.DRG分组工具在医院绩效评价、流行病学和卫生经济学等领域的应用研究。DRG未来研究趋势主要集中于:a.基于DRG分组信息的流行病学等领域的研究;b.对于捆绑支付改革效果的研究;c.国家层面和国家间基于DRG的付费系统的比较和分析。

Objective To analyze and summarize the research progress and trends in the international DRG field,and provide a new perspective and reference for China’s DRG research.Methods Based on 2473 research publications on DRG from 1981 to 2019 in the core collection of Web of Science database,a bibliometric analysis was conducted on the number of publications,the distribution of publishing institutions,carrier journals and contributors’ countries(regions),co-authorship status,hotspots and trends by using the Citespace map analysis and VOSviewer analysis software.Results & Conclusion The research results show the number of international DRG research publications has fluctuated and increased from 1981 to 2019,and DRG research has a sustained popularity worldwide.Besides,research on DRG in the US has been in the lead.The research hotspots are mainly reflected in three aspects:a.Study on the development and evaluation of patient grouping tools based on severity of illness.b.Study on the effect of DRG-PPS on the performance of medical institutions.c.Study on the application of DRG grouping tool in hospital performance evaluation,epidemiology and health economics.The future research trends of DRG mainly focus on:a.Epidemiology based on DRG grouping information;b.Research on the effect of bundled payment reform;c.Comparison and analysis of DRG-based payment systems at the national level and among different countries.
