哮喘缓解期患儿的心肺运动试验分析 被引量:3

摘要 目的探讨哮喘缓解期儿童与正常健康儿童心肺运动试验(cardiopulmonary exercise test,CPET)、肺功能各项指标的差异,以及有氧运动对哮喘缓解期儿童心肺功能的影响。方法选取2016年8月至2017年7月就诊于北部战区总医院儿科的7~14岁哮喘缓解期儿童50例为研究组,健康体检儿童50例为对照组,完善静态肺功能、CPET检测。并且将哮喘缓解期患儿按照是否进行有氧运动分为运动组(23例)及非运动组(27例),随访3个月后再次进行静态肺功能以及CPET检测。结果哮喘缓解期组无氧阈时公斤摄氧量(maximum oxygen consumption at anaerobic threshold,VO_(2)/kg@AT)、最大公斤摄氧量(maximum oxygen consumption,VO_(2)max/kg)、最大心率(maximum heart rate,HRmax)、最大耐受功率(maximal tolerance power,Pmax)较健康对照组明显降低,两组差异有统计学意义(P均<0.05)。3个月有氧运动后,运动组VO_(2)/kg@AT[(19.95±2.46)ml/(min·kg)]较非运动组[(16.55±4.12)ml/(min·kg)]明显升高,差异有统计学意义(P<0.05)。运动组VO_(2)max/kg[(30.21±5.78)ml/(min·kg)]较非运动组[(25.59±6.63)ml/(min·kg)]明显升高,差异有统计学意义(P<0.05)。结论哮喘缓解期儿童心肺功能低于正常健康儿童。适当有氧运动后,哮喘缓解期儿童心肺功能有所改善。 Objective To investigate the differences of cardiopulmonary exercise test(CPET)between children with remission of asthma and normal children and effects of aerobic exercise on cardiopulmonary function in asthmatic children in remission.Methods Fifty cases of children diagnosed with remission of asthma and fifty cases of children with healthy physical examination from 7 to 14 years old in General Hospital of Northern Theater Command from August 2016 to July 2017 were selected as the study group and the control group to collect static lung function and CPET.In addition,children with asthma in remission were divided into exercise group(23 cases)and non-exercise group(27 cases)according to whether they performed aerobic exercise.After 3 months of follow-up,static lung function and CPET were performed again.Results In the remission of asthma group,VO_(2)/kg@AT,VO_(2)max/kg,HRmax,Pmax were lower than those of healthy control group and the difference between the two groups was statistically significant(P<0.05).The aerobic exercise group VO_(2)/kg@AT[(19.95±2.46)ml/(min·kg)]was higher than that without aerobic exercise group[(16.55±4.12)ml/(min·kg)].The aerobic exercise group VO_(2)max/kg[(30.21±5.78)ml/(min·kg)]was higher than that without aerobic exercise group[(25.59±6.63)ml/(min·kg)],and the difference between the two groups was statistically significant(P<0.05).Conclusion Cardiopulmonary exercise test in children with asthma in remission is different from normal healthy children.After proper aerobic exercise,the cardiopulmonary function of children in asthma remission period has been improved.
