B超、X线对手足外伤后非金属异物存留的诊断价值

Diagnositic efficacy and comparison between X-ray and B ultrasonography on nonmetal foreign bodies retention in hand and foot injuries

目的比较分析X线、B超等辅助检查手段对于手足外伤后可疑非金属异物存留病例的术前诊断价值和手术指导意义。方法选取2015年1月-2019年5月收治的137例手足外伤后可疑非金属异物存留患者的临床资料,回顾性分析患者B超检查及X线检查结果,结合手术结果,对比不同检查方法的图像表现与诊断价值。结果B超结果与手术结果一致性较好(Kappa=0.964,P<0.01),X线结果与手术结果一致性较差(Kappa=0.219,P<0.01);B超检查灵敏度达100%,X线对草木刺、鱼骨等材质的异物灵敏度较低。结论对于非金属异物B超检查有其独特优势,应作为常规检查,以降低漏诊率。

Objective To analyze the result of X-ray and Ultrasonography on detecting foreign bodies in hand and foot injuries,to compare the diagnositic efficacy and significance for surgery.Methods Imaging examination results of 137 patients with suspected foreign bodies in the hand and foot were analyzed.Diagnositic efficacy of the two kind of examination were evaluated and compared according to the surgical findings.Results Consistency between surgical findings and Ultrasonography results were better than that X-ray results.The sensibility of Ultrasonography reached 100%while sensibility of X-ray were proved to decrease in detecting wood and fishbone.Conclusion Ultrasonography is reliable for diagnosing and localizing nonmetal foreign bodies.
