摘要
目的:探讨胰岛素样生长因子-1(IGF-1)在2型糖尿病周围神经病变(DPN)中的作用及可能的机制。方法:选择2型糖尿病(T_(2)DM)患者84例,根据超声结果分为单纯T_(2)DM组36例,DPN组48例,分别检测两组的IGF-1、空腹血糖(FPG)、胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、糖化血红蛋白(HbA1c)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、碱性磷酸酶(ALP)、总胆汁酸(TBA)、同型半胱氨酸(HCY)、乳酸、尿酸(UA)水平,通过两组间t检验或χ^(2)检验比较两组间各项一般资料及实验室检查指标差异,同时通过pearson相关性分析DPN组患者IGF-1水平与其他实验室检查指标的相关性。结果:DPN组较T 2DM组相比,IGF-1、UA水平降低,差异有统计学意义(P<0.05),HbA1c、TC、LDL-C、HCY、乳酸水平升高,差异有统计学意义(P<0.05)。IGF-1与UA呈正相关(r=0.349,P=0.021),与HCY呈负相关(r=-0.222,P=0.042),差异均有统计学意义(P<0.05)。结论:DPN患者IGF-1水平降低,提示IGF-1可能通过多种机制参与DPN的发生、发展。
Objective To explore the role and possible mechanism of insulin-like growth factor-1(IGF-1)in type 2 diabetic peripheral neuropathy(DPN).Method 84 patients with type 2 diabetes(T_(2)DM)were selected.According to whether to merge DPN,they were divided into 36 cases in the simple T_(2)DM group and 48 cases in the DPN group.IGF-1,fasting blood glucose(FPG),cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL-C),high density lipoprotein(HDL-C),glycated hemoglobin(HbA1c),gluten Alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),total bile acid(TBA),homocysteine(HCY),lactic acid,uric acid(UA)levels were detected in two groups.The differences in general indicators and biochemical indicators between the two groups were compared by the t test orχ^(2)test between the two groups.Pearson correlation analysis was used to analyze the correlation between IGF-1 levels and other biochemical indicators in the DPN group.Results Compared with T_(2)DM group,DPN group had lower levels of IGF-1 and UA,the difference was statistically significant(P<0.05),and higher levels of HbA1c,TC,LDL-C,HCY and lactate,the difference was statistically significant(P<0.05).IGF-1 was positively correlated with UA(r=0.349,P=0.021),and negatively correlated with HCY(r=-0.222,P=0.042),the difference was statistically significant(P<0.05).Conclusion The level of IGF-1 in DPN patients is reduced,suggesting that IGF-1 plays an important role in the occurrence and development of DPN.
