PDGF-B和PDGFR-β蛋白在狼疮性肾炎中的表达及临床意义 被引量:1

Expression and clinical significance of PDGF-B and PDGFR-βprotein in lupus nephritis

摘要 目的探讨血小板衍生生长因子(PDGF)-B及β型血小板衍生生长因子受体(PDGFR-β)蛋白在狼疮性肾炎(LN)中的表达及其临床意义。方法收集2018年10月至2019年12月安徽医科大学第一附属医院风湿免疫科住院的系统性红斑狼疮(SLE)患者64例(LN组34例,SLE非LN组30例)作为研究对象。另选取该院健康体检中心体检健康者28例作为对照组。分别采用免疫组化法及酶联免疫吸附试验法检测肾脏组织及外周血PDGF-B和PDGFR-β蛋白的表达水平。比较肾脏组织及外周血PDGF-B和PDGFR-β蛋白表达水平,并分析其与SLE疾病活动指数(SLEDAI)评分、补体C3、C4水平、超敏C反应蛋白(hs-CRP)、抗双链DNA(ds-DNA)抗体水平、24 h尿蛋白定量(24 h UPQ)、肌酐(Cr)、尿素氮(BUN)、估算肾小球滤过率(eGFR1)和抗C1q抗体的相关性。结果LN组、SLE非LN组肾脏组织及外周血中PDGF-B和PDGFR-β蛋白表达水平与对照组比较,差异有统计学意义(P<0.05)。LN患者肾脏组织中PDGF-B和PDGFR-β蛋白表达水平与抗C1q抗体呈正相关(P<0.05),与eGFR1呈负相关(P<0.05)。LN患者肾脏组织和外周血中PDGF-B与PDGFR-β蛋白表达水平均呈正相关(肾脏组织r=0.663,P<0.001;外周血r=0.547,P=0.001)。结论PDGF-B和PDGFR-β蛋白参与了SLE肾损伤的发病机制,对LN判断具有重要临床价值。 Objective To investigate the expression of platelet-derived growth factor(PDGF)-B and platelet-derived growth factor receptor(PDGFR-β)protein in lupus nephritis(LN)and its clinical significance.Methods A total of 64 patients with systemic lupus erythematosus(SLE)admitted to the department of rheumatism and immunization of the first affiliated hospital of Anhui medical university from October 2018 to December 2019(34 in the LN group and 30 in the SLE non-LN group)were enrolled as subjects.Another 28 healthy subjects from the health examination center of the hospital were selected as control group.The expression levels of PDGF-B and PDGFR-βprotein were detected by immunohistochemistry and enzyme-linked immunosorbent assay in kidney tissues and peripheral blood.The expression levels of PDGF-B and PDGFR-βprotein in kidney tissues and peripheral blood were compared.The correlations of SLE disease activity index(SLEDAI)score,C3,C4 levels,hypersensitive C-reactive protein(hs-CRP),anti-double-strand DNA(ds-DNA)antibody levels,24 h urinary protein quantification(24 h UPQ),creatinine(Cr),blood urea nitrogen,estimated glomerular filtration rate(eGFR1)and anti-C1q antibody were analyzed.Results The expression levels of PDGF-B and PDGFR-βprotein in kidney tissues and peripheral blood of LN group and SLE non-LN group were compared with those of control group,and the difference was statistically significant(P<0.05).The expression levels of PDGF-B and PDGFR-βprotein in kidney tissues of patients with LN were positively correlated with anti-C1q antibody(P<0.05),and negatively correlated with eGFR1(P<0.05).The expression levels of PDGF-B and PDGFR-βprotein in kidney tissues and peripheral blood of patients with LN were positively correlated(renal tissue r=0.663,P<0.001;peripheral blood r=0.547,P=0.001).Conclusion PDGF-B and PDGFR-βprotein are involved in the pathogenesis of renal damage in SLE,and have important clinical value in the diagnosis of LN.
