摘要
目的评价细胸腔引流管接负压球在单操作孔胸腔镜肺癌根治术后引流中的应用价值。方法选取2017年6月至2019年12月于医院胸外科行单操作孔胸腔镜肺癌根治术的100例患者,按照随机数字表法分为对照组和试验组,各50例。对照组术后放置28F传统硅胶下胸腔引流管接水封瓶,试验组术后放置15F螺旋十字形硅胶软质下胸腔引流管接负压球,比较两组胸腔总引流量、下胸腔引流管留置时间、术后住院时间、再次胸腔置管率、拔管后气胸发生率、下胸腔引流管切口愈合不良发生率及术后疼痛程度。结果两组胸腔总引流量、下胸腔引流管留置时间、术后住院时间、再次胸腔置管率、拔管后气胸发生率及术后第1、7天视觉模拟评分法(VAS)评分比较,差异均无统计学意义(P>0.05);试验组下胸腔引流管切口愈合不良发生率及术后第4天VAS评分均低于对照组,差异有统计学意义(P<0.05)。结论细胸腔引流管接负压球用于单操作孔胸腔镜肺癌根治术后引流中安全、有效,且能减少发生引流管切口愈合不良,减轻术后疼痛程度。
Objective The application value of thin thoracic drainage tube connected with negative pressure ball in drainage after single-port thoracoscopic radical resection of lung cancer was evaluated.Methods A total of 100 patients who underwent single-port thoracoscopic radical resection of lung cancer in the department of thoracic surgery of hospital from June 2017 to December 2019 were selected and they were divided into a control group and a test group according to the random number table method,with 50 cases in each.After operation,the control group were placed with 28F traditional silica gel louer thoracic drainage tube connected with a water-sealed bottle,while the test group were placed with 15F spiral cruciform silica gel soft louer thoracic drairage tube connected with a negative pressure ball.The total pleural drainage volume,pleural drainage tube indwelling time,postoperative hospital stay,re-thoracic catheterization rate,occurrence of pneumothorax after extubation,incidence of poor healing of lower pleural drainage tube incision and the degree of postoperative pain were compared between the two groups.Results When the total pleural drainage volume,pleural drainage tube indwelling time,postoperative hospital stay,re-thoracic catheterization rate,occurrence of pneumothorax after extubation,and visual analogue scale(VAS)scores on the first and seventh days after surgery were compared,the differences were not statistically significant(P>0.05);The incidence of poor healing of pleural drainage tube incision and the VAS scores on the 4th day after surgery in the test group were both lower than those in the control group,the differences were statistically significant(P<0.05).Conclusions It was safe and effective that when thin thoracic drainage tube connected with negative pressure ball was applied for drainage after single-port thoracoscopic radical resection of lung cancer.It showed that poor healing of pleural drainage tube incision cauld be reduced and postoperative pain could be alleviated meanwhile.
