微创钻孔引流术与开窗血肿清除术治疗高血压脑出血疗效观察

Observation of Curative Effect of Minimally Invasive Drilling and Drainage and Fenestration Hematoma Removal for Hypertensive Cerebral Hemorrhage

目的对比分析微创钻孔引流术与开窗血肿清除术治疗高血压脑出血的疗效。方法选取2015年1月至2020年5月江阴市中医院收治的92例高血压脑出血患者为研究对象,按照随机数字表法分为两组,各46例。对照组行开窗血肿清除术治疗,观察组行微创钻孔引流术治疗。对比两组疗效、手术前后美国国立卫生研究院卒中量表(NIHSS)评分、日常生活能力量表(ADL)评分及并发症发生率。结果观察组疗效优于对照组(P<0.05);观察组术后2周NIHSS评分低于对照组,术后3个月ADL评分高于对照组,并发症总发生率较对照组低(P<0.05)。结论微创钻孔引流术治疗高血压脑出血患者疗效优于开窗血肿清除术,可减轻神经功能缺损,提升日常生活能力,减少并发症。

Objective To compare the efficacy of minimally invasive drilling and drainage and fenestration hematoma removal in the treatment of hypertensive cerebral hemorrhage.Methods A total of 92 patients with hypertensive intracerebral hemorrhage admitted to Jiangyin Hospital of Traditional Chinese Medicine from January 2015 to May 2020 were selected as the research objects and divided into two groups according to random number table method with 46 cases in each group.The control group was treated with fenestrated hematoma evacuation,and the observation group was treated with minimally invasive drilling and drainage.Efficacy,National Institutes of Health Stroke Scale(NIHSS)scores,ADL scores before and after surgery,and complication rates were compared between the two groups.Results The efficacy of the observation group was better than that of the control group(P<0.05),the observation group had lower NIHSS scores at 2 weeks after surgery than the control group,higher ADL scores at 3 months after surgery than the control group,and a lower total incidence of total complications than the control group(P<0.05).Conclusion The effect of minimally invasive drilling and drainage in the treatment of patients with hypertensive cerebral hemorrhage is better than fenestration hematoma removal,which can reduce neurological deficits,improve the ability of daily living,and reduce complications.
