一氧化氮吸入联合西地那非口服治疗新生儿持续肺动脉高压的研究

摘要 目的分析一氧化氮(NO)吸入联合西地那非口服治疗新生儿持续肺动脉高压的效果。方法选取开远市人民医院2018年4月至2020年3月收治的30例新生儿持续肺动脉高压患儿为研究对象。采用随机数字法分为对照组(常规治疗+西地那非)与观察组(同时联合NO吸入治疗),每组15例。比较两组患儿疗效及治疗24、48和72 h后的肺动脉压(PAP)。对比两组患儿机械通气时间和住院时间。结果观察组的治疗有效率显著高于对照组,为93.33%,差异有统计学意义(χ^(2)=6.136,P<0.05);观察组治疗24 h PAP是(75.32±3.28)mm Hg,48 h PAP是(61.02±2.53)mm Hg,72 h PAP是(43.98±3.28)mm Hg,低于对照组(t=3.302/7.324/3.753,P<0.05);观察组机械通气时间(80.23±15.02)h,住院时间(12.43±3.27)d,低于对照组(t=7.382/5.751,P<0.05)。结论NO吸入联合西地那非口服治疗新生儿持续肺动脉高压可以获得理想效果,缩短机械通气时间,加快康复,改善肺动脉高压,值得在临床推广使用。 Objective To analyze the effect of nitric oxide(NO)inhalation combined with sildenafil in the treatment of neonatal persistent pulmonary hypertension.Methods A total of 30 cases of neonatal persistent pulmonary hypertension in Kaiyuan People's Hospital from April 2018 to March 2020 were selected as the research objects.The patients were randomly divided into control group(conventional treatment+sildenafil)and observation group(combined with NO inhalation therapy),15 cases in each group.Compare the efficacy and pulmonary artery pressure after 24、48 and 72 h(PAP).The mechanical ventilation time and hospitalization time of the two groups were compared.Results The effective rate of the observation group was significantly higher than that of the control group,which was 93.33%,the difference was statistically significant(χ^(2)=6.136,P<0.05);pulmonary arterial pressure(PAP)of observation group treated for 24 h was(75.32±3.28)mm Hg,48 h PAP was(61.02±2.53)mm Hg,72 h PAP was(43.98±3.28)mm Hg,which was lower than that in the control group(t=3.302/7.324/3.753,P<0.05);the duration of mechanical ventilation and hospital stay in the observation group were(80.23±15.02)h and(12.43±3.27)d,which were lower than those in the control group(t=7.382/5.751,P<0.05).Conclusion NO inhalation combined with sildenafil in the treatment of neonatal persistent pulmonary hypertension can obtain ideal effect,shorten the time of mechanical ventilation,accelerate rehabilitation,improve pulmonary hypertension,which is worthy of clinical application.
