超声引导下微创介入联合抗生素冲洗治疗乳腺脓肿的临床效果

Clinical effect of ultrasound-guided minimally invasive intervention combined with antibiotic irrigation in the treatment of breast abscess

目的探讨超声引导下微创介入联合抗生素冲洗治疗乳腺脓肿的临床效果。方法选取我院2018年1月至2020年1月收治的68例乳腺脓肿患者,按照手术方式将其分为观察组(超声引导下微创介入联合抗生素冲洗)和对照组(脓肿切开引流术),各34例。比较两组的临床效果。结果观察组手术时间、切口长度、脓肿愈合时间均短于对照组,术中出血量小于对照组,总换药次数少于对照组(P<0.05)。术后,两组CRP、IL-6、P物质、IL-8水平及VAS评分均低于术前,且观察组低于对照组(P<0.05)。观察组并发症总发生率及乳腺脓肿复发率均低于对照组(P<0.05)。结论超声引导下微创介入联合抗生素冲洗能降低乳腺脓肿患者炎症因子水平,减少手术创伤,促进术后恢复,值得临床推广。

Objective To investigate the clinical effect of ultrasound-guided minimally invasive intervention combined with antibiotic irrigation in the treatment of breast abscess.Methods A total of 68 patients with breast abscess admitted in our hospital from January 2018 to January 2020 were selected and divided into observation group(ultrasound-guided minimally invasive intervention combined with antibiotic irrigation)and control group(abscess incision and drainage),with34 cases in each group.The clinical effects of the two groups were compared.Results The operation time,incision length and abscess healing time of the observation group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the total dressing change frequency was less than that of the control group(P<0.05).After operation,CRP,IL-6,substance P,IL-8 levels and VAS scores of the two groups were lower than those before operation,and those of the observation group were lower than the control group(P<0.05).The total incidence of complications and recurrence rate of breast abscess of the observation group were lower than those of the control group(P<0.05).Conclusion Ultrasound-guided minimally invasive intervention combined with antibiotic irrigation can reduce the levels of inflammatory factors in patients with breast abscess,reduce operation trauma,promote postoperative recovery,which is worthy of clinical promotion.
