MRI-DWI用于脊柱结核、转移性肿瘤的鉴别的成像效果及准确性分析

摘要 目的探讨磁共振扩散加权成像(MRI-DWI)用于脊柱结核、转移性肿瘤的鉴别的成像效果及准确性。方法选取收治的36例脊柱结核患者作为脊柱结核组,47例脊柱转移性肿瘤患者作为脊柱转移性肿瘤组,全部患者均行MRI-DWI,比较两组的DWI信号强度,在选定兴趣区后测量和比较两组不同组织表观弥散系数(ADC)值,以病理结果为金标准评价MRI-DWI诊断脊柱转移性肿瘤的效能。结果两组患者的DWI信号强度比较差异显著(P<0.05);脊柱转移性肿瘤组患者骨髓水肿、椎体病灶、椎旁病变组织表观弥散系数(ADC)值低于脊柱结核组(P<0.05);MRI-DWI诊断脊柱转移性肿瘤的特异性为(94.44%),敏感性为(93.62%),准确性为(93.98%),误诊率为(6.38%),漏诊率为(5.56%);MRI-DWI诊断脊柱转移性肿瘤的准确性与金标准比较无明显差异(P>0.05)。结论在脊柱结核、转移性肿瘤鉴别中应用MRI-DWI检查诊断结果可靠,与病理结果具有较好的一致性。
