补脾益肾扶正固本方联合泼尼松治疗儿童急性肾小球肾炎对肾功能、免疫功能的影响

The effects of the Bupi Yishen Fuzheng Guben prescription plus prednisone on renal function and immune function in children with acute glomerulonephritis

目的:探讨补脾益肾扶正固本方联合泼尼松治疗儿童急性肾小球肾炎(Acute Glomerulonephritis,AGN)对肾功能、免疫功能的影响。方法:选取本院于2018年4月1日-2020年4月1日收治的水湿浸渍证AGN患儿97例,按随机数字表法分为治疗组(50例)与对照组(47例),两组入院后均给予常规对症治疗,在此基础上,对照组给予泼尼松治疗,治疗组在对照组的基础上采用补脾益肾扶正固本方治疗。治疗4周后评价两组临床疗效、症状缓解时间、肾功能及免疫功能改善情况。结果:治疗组临床治疗总有效率明显高于对照组(P<0.05);治疗组水肿消退时间、血尿消退时间、血沉恢复时间、尿常规恢复时间均明显短于对照组(P<0.05);治疗前两组肾功能指标血尿素氮(Blood Urea Nitrogen,BUN)、血肌酐(Serum Creatinine,Scr)、24 h尿蛋白定量(24 hours Urine Protein,24 h Upro)、1 h尿红细胞排泄率(1 hour Urine Red Blood Cell Excretion Rate,1 h URBCer)水平比较无明显差异(P>0.05),治疗后两组各项肾功能指标较治疗前明显降低,治疗组各项指标明显低于对照组(P<0.05);治疗前两组免疫功能指标免疫球蛋白(Immunoglobulin,Ig)A、IgG水平比较无明显差异(P>0.05),治疗后两组IgA、IgG水平较治疗前明显提高,治疗组IgA、IgG水平显著高于对照组(P<0.05)。结论:针对急性AGN患儿给予补脾益肾扶正固本方联合泼尼松治疗效果确切,可促进患儿症状缓解,改善肾功能,增强机体免疫力,值得临床推广应用。

Objective:To explore the effects of the Bupi Yishen Fuzheng Guben prescription(补脾益肾扶正固本方)plus prednisone on renal function and immune function of children with AGN.Methods:97 cases were selected and divided into the treatment group(50 cases)and the control group(47 cases).Both groups were given conventional symptomatic treatment after admission.On this basis,the control group was given prednisone treatment,and the treatment group was treated with the Bupi Yishen Fuzheng Guben prescription more.Results:The total efficiency in the treatment group was significantly higher than the control group(P<0.05).The edema resolution time,hematuria resolution time,erythrocyte sedimentation recovery time,and urine routine recovery time in the treatment group were significantly fewer than the control group(P<0.05).After treatment,the BUN,Scr,24 hours Upro and 1 hour uRBCer in the groups were significantly lower than before treatment,and those in the treatment group were significantly lower than the control group(P<0.05),the levels of IgA and IgG in the groups were higher than before treatment,and those in the treatment group were significantly higher than the control group(P<0.05).Conclusion:The Bupi Yishen Fuzheng Guben prescription plus prednisone on renal function and immune function of children with AGN is effective,can relieve clinical symptoms,improve renal function,strengthen the body’s immunity,and is worthy of clinical application.
