掌侧单切口扩展入路治疗桡骨远端AO-C型骨折的效果观察 被引量:1

摘要 目的探讨经掌侧单切口扩展入路与掌背侧联合入路单纯锁定钢板固定治疗桡骨远端AO-C型骨折的临床效果。方法收治的桡骨远端AO-C型骨折患者63例,分别采用经掌侧单切口扩展入路(观察组)与掌背侧联合入路(对照组)单纯锁定钢板固定治疗手术治疗。术后随访6~8个月,采用VAS评分评价腕关节的疼痛,改良Gartland和Werley评分(GW评分)评价腕关节功能,采用Sarmiento放射学评分评价疗效。结果 63例患者均获得完整随访,所有患者无切口感染及骨不连的发生,无内固定松动,无背侧肌腱激惹。观察组患者术后VAS评分为[2.00(2.00,3.00)]分,对照组为[2.50(2.00,4.00)]分,末次随访时观察组患者改良Sarmiento评分优良率100%。术后3个月改良Garland-Werley评分为(4.96±2.32)分,优良率为88.10%;对照组改良Garland-Werle评分为(5.35±2.57)分,优良率80.95%。术后6个月观察组改良Garland-Werley评分为(4.88±2.14)分,优良率为92.9%;对照组改良Garland-Werley评分为(4.98±2.36)分,优良率90.48%。结论经掌侧单切口扩展入路治疗桡骨远端AO-C型骨折能够有效地治疗复杂的桡骨远端关节内骨折,保护了背侧血运及合页结构,避免掌背侧联合入路的缺点。
