甲磺酸多沙唑嗪在输尿管镜手术治疗输尿管上段结石围术期中应用的效果

Application effect of perioperative doxazosin mesylate in ureteroscopic surgery for upper ureteral calculi

目的观察甲磺酸多沙唑嗪在输尿管镜手术治疗输尿管上段结石围术期中应用的效果。方法将110例输尿管上段结石患者随机分为观察组与对照组,各55例。两组患者均常规行输尿管镜手术,观察组术前3 d至术后7 d给予口服甲磺酸多沙唑嗪。比较两组患者手术情况、输尿管支架管相关症状问卷(USSQ)评分及结石清除率,并记录观察组用药期间药物不良反应发生情况。结果观察组一次手术成功率高于对照组,手术时间、术中肾盂压力短于或低于对照组(均P<0.05);术后1周,观察组USSQ调查表各项评分均低于对照组,且结石清除率高于对照组(均P<0.05)。观察组用药期间未出现明显的与药物相关的不良反应。结论对于输尿管上段结石患者,在输尿管镜手术围术期应用甲磺酸多沙唑嗪可以提高手术成功率和短期排石率,减轻手术损伤和术后疼痛及输尿管支架相关症状。

Objective To observe the application effect of perioperative doxazosin mesylate in the ureteroscopic surgery for upper ureteral calculi.Methods A total of 110 patients with upper ureteral calculi were randomly divided into observation group and control group,with 55 cases in each group.Both groups underwent routine ureteroscopic surgery,the observation group was given oral doxazosin mesylate from three days preoperatively to seven days postoperatively.Operation condition,the Ureteral Stent Symptom Questionnaire(USSQ)score,and stone clearance rate were compared between the two groups,and the incidence of adverse drug reactions during the medication period was observed in the observation group.Results The observation group had a higher success rate of single operation,shorter operation duration and lower intraoperative renal pelvic pressure than the control group(all P<0.05);one week after operation,the observation group reported lower scores of all items on the USSQ questionnaire,and a higher stone clearance rate as compared with the control group(all P<0.05).No prominent adverse drug-related reactions occurred in the observation group during the medication period.Conclusion For patients with upper ureteral calculi,the application of doxazosin mesylate in the perioperative period of ureteroscopic surgery can improve the success rate of the operation and the rate of short-term stone removal,ameliorate operation trauma and postoperative pain and ureteral stent-associated symptoms.
