分析基于肝胆动态显像评估肝功能对肝胆摄取及排泌的影响

摘要 目的:研究基于肝胆动态显像评估肝功能对肝胆摄取及排泌的影响。方法:回顾性分析本院2014年1月—2020年6月接受诊断的胆囊运动功能异常患者71例纳入胆囊运动功能异常组,将其根据肝功能是否异常分为(肝功能正常组44例,肝功能异常组27例),根据胆囊运动功能障碍的表现不同分为(胆囊充盈缓慢组27例、胆囊充盈障碍组18例、胆囊排空功能低下组26例),另选同期接受显像的健康体检者12例纳入对照组。对2组患者均进行肝胆动态显像评估,观察对比2组肝脏、胆囊摄取及排泌功能相关指标等。结果:相较于对照组,胆囊运动功能异常组受检者的T1/2、Tmax更长(P<0.05),胆囊充盈缓慢组和排空功能低下组的胆囊开始显影时间更长(P<0.05)。在胆囊运动功能异常组71例受检者中,肝功能正常组与肝功能异常组T1/2指标差异无统计学意义(P>0.05)。相较于肝功能正常组,肝功能异常组受检者的Tmax更长,胆囊充盈缓慢组和排空功能低下组中(共53例)肝功能异常的受检者胆囊开始显影时间更长(P<0.05);相较于对照组,胆囊排空功能低下组受检者的GBEF指标更低(P<0.05)。结论:肝胆动态显像评估肝功能对肝胆摄取及排泌有着重要影响,对胆囊运动功能异常患者进行功能评价有重要的意义。 Objective To study the influence of based on hepatobiliary dynamic imaging in assessment of liver function on hepatobiliary uptake and excretion.Methods 71 cases of patients with abnormal gallbladder motility from January 2014 to June 2020 were included in the gallbladder motility abnormality group,and the patients were divided into normal liver function group(44 cases)and abnormal liver function group(27 cases)according to whether the liver function is abnormal.And other 12 cases of healthy people who were diagnosed in the same period were selected as the control group.Then,the liver and gallbladder dynamic imaging between two groups were evaluated,and the liver and gallbladder function related indexes of two groups were observed.Results The T1/2 and Tmax of the subjects in the abnormal gallbladder movement function group were longer than that in the control group(P<0.05).The gallbladder began to develop longer between the gallbladder filling slowly group and emptying function low group(P<0.05).71 subjects in the abnormal gallbladder movement group,and there was no significant difference in T1/2 between normal liver function group and abnormal liver function group(P>0.05).The Tmax of patients with abnormal liver function was longer than that in the normal liver function group,and the 53 patients with abnormal liver function in the slow filling gallbladder group and the low emptying function group,the gallbladder began to develop longer(P<0.05);The GBEF index in the low gallbladder emptying function group was lower than that in the control group(P<0.05).Conclusion The dynamic imaging of liver and gallbladder plays an important role in liver and gallbladder uptake and excretion,and it is great significance to evaluate the function of gallbladder in patients with abnormal motor function.
