二维超声TI-RADS分类联合超声造影对甲状腺微小癌的诊断价值 被引量:3

摘要 目的:探讨甲状腺影像报告与数据系统(TI-RADS)分类、超声造影(CEUS)在诊断甲状腺微小癌中的应用价值。方法:选取2019年7月—2020年6月于我院就诊的65例甲状腺微小结节患者共74个结节,于术前均进行TI-RADS分类和CEUS检查,并以手术病理结果为"金标准",计算TI-RADS 分类、CEUS单独及二者联合诊断甲状腺微小癌的敏感度、特异度、阳性预测值、阴性预测值、准确率。结果:74个结节中,经手术病理确诊恶性结节35个,TI-RADS分类检出恶性结节51个,CEUS检出甲状腺微小癌43个。TI-RADS分类单独诊断甲状腺微小癌的特异度较低,仅46.15%。TI-RADS分类联合CEUS诊断甲状腺微小癌的敏感度、特异度、阳性预测值、阴性预测值、准确率分别为91.43%、74.36%、76.19%、90.63%、82.43%均高于TI-RADS 分类(85.71%、46.15%、58.82%、78.26%、64.86%)和CEUS(85.71%、66.67%、69.77%、83.87%、75.68%)单独诊断,差异具有统计学意义(P<0.05)。结论:TI-RADS分类、CEUS在诊断甲状腺微小癌中均有一定的价值,TI-RADS分类单独诊断的特异度较低,二者联合在诊断甲状腺微小癌中具有更高的效能。
