抑郁症患者心理韧性水平与血清BDNF、IL-6水平的相关性 被引量:4

Correlation between the level of mental resilience and serum BDNF and IL-6 levels in patients with depression

摘要 目的:观察抑郁症患者心理韧性水平及血清脑源性神经生长因子(BDNF)、白细胞介素-6(IL-6)水平,分析心理韧性水平与BDNF、IL-6水平的关系,指导抑郁症心理韧性评估与干预。方法:选取2018年10月-2019年10月本院接收的160例抑郁症患者,将其设为抑郁症组,根据汉密尔顿抑郁量表(HAMD)评估抑郁程度并分组;另选取同期本院体检中心经全身体检证实为健康的160例健康体检人群作为对照组;采用心理韧性量表(CD-RISC)评估受检者心理韧性水平,并检测受检者血清BDNF、IL-6水平;分析抑郁症患者心理韧性水平与血清BDNF、IL-6水平的相关性。结果:抑郁症组CD-RISC评分、BDNF水平低于对照组,血清IL-6水平高于对照组,差异有统计学意义(t=19.285,11.220,17.754;P<0.05);160例抑郁症患者经HAMD量表评估结果显示,有40例轻度、77例中度、43例重度;中度组、重度组CD-RISC评分、BDNF水平均低于轻度组,IL-6水平高于轻度组;且重度组CD-RISC评分、BDNF水平低于中度组,IL-6高于中度组(F=83.730,34.448,63.350;P<0.01);相关性检验结果显示,抑郁症患者CD-RISC评分与血清BDNF水平呈正相关,与血清IL-6水平呈负相关(r=0.395,-0.401;P<0.01);160例抑郁症患者中,心理韧性较好(CD-RISC评分≥60分)124例,心理韧性较差(CD-RISC评分<60分)36例;心理韧性较好组血清BDNF水平高于较差组,且IL-6水平低于较差组(t=4.438,-5.536;P<0.05);经Logistics回归分析结果显示,血清BDNF、IL-6水平是抑郁症患者心理韧性水平的影响因素。结论:抑郁症患者伴不同程度心理韧性水平下降,这种下降情况与下降程度可能受患者血清BDNF、IL-6水平影响,考虑早期持续监测抑郁症患者血清BDNF、IL-6水平,对评估患者心理韧性水平有一定价值。 Objective:To observe the level of mental resilience and serum brain-derived nerve growth factor(BDNF)and interleukin-6(IL-6)levels in patients with depression,to analyze the relationship between mental resilience and BDNF and IL-6 levels,and to guide the evaluation and intervention of psychological resilience in depression.Methods:A total of 160 patients with depression who were received by our hospital from October 2018 to October 2019 were selected as the depression group,and the degree of depression was evaluated and grouped according to the Hamilton Depression Scale(HAMD);160 healthy people who were confirmed to be healthy by physical examination in our hospital were used as the control group at the same period;the psychological resilience scale(CD-RISC)was used to assess the psychological resilience of the subjects,and the serum BDNF and IL-6 levels of the subjects were detected;We analyzed the correlation between patients’ psychological resilience and serum BDNF and IL-6 levels.Results:The CD-RISC score and BDNF level in the depression group were lower than those in the control group,and the serum IL-6 level was higher than that in the control group,the difference was statistically significant(t=19.285,11.220,17.754;P<0.05);160 depression patients were evaluated by the HAMD scale and it showed that there were 40 cases of mild,77 moderate,and 43 severe;the CD-RISC score and BDNF level of the moderate group and severe group were lower than that of the mild group,and the level of IL-6 was higher than that of the mild group;CD-RISC score and BDNF level of the severe group were lower than moderate group,IL-6 was higher than moderate group(F=88.370,34.448,63.350;P<0.05);Correlation test results showed that CD-RISC score of depression patients was positively correlated with serum BDNF level(r=0.395,P<0.05),negatively correlated with serum IL-6 level(r=-0.401,P<0.05);Of the 160 cases of patients with depression,124 cases had good mental toughness(CD-RISC score ≥60 points)and 36 cases had poor mental toughness(CD-RISC score<60 points);Serum BDNF level in the group with better mental resilience was higher than that in the poor group,and IL-6 level was lower than that in the poor group(t=4.438,-5.536;P<0.05);Logistics regression analysis showed that the serum BDNF and the levels of IL-6 were the influencing factors of psychological resilience in patients with depression(OR>1,P<0.05).Conclusion:Depression patients are accompanied by different levels of mental resilience.This condition and the degree of decline may be affected by the serum BDNF and IL-6 levels of patients.Early and continuous monitoring of serum BDNF and IL-6 levels should be considered in patients with depression and it has certain value on the evaluation of mental resilience level.
