新型冠状病毒肺炎疫情期间肿瘤患者心理困扰及影响因素

Analysis of psychological distress and related factors of tumor patients during the epidemic of novel coronavirus pneumonia

目的:调查新冠肺炎流行期间肿瘤患者心理困扰现况及其影响因素。方法:本研究为横断面描述性研究,采用便利抽样方法选取广东省5所三甲医院189例肿瘤患者,应用一般资料调查表、凯勒斯心理困扰量表(K10)、简易应对方式问卷(SCSQ)进行网络问卷调查。结果:新冠肺炎流行期间肿瘤患者心理困扰的检出率为50.79%;单因素分析显示,躯体状况包括睡眠状况、饮食状况、体重状况、疼痛状况在心理困扰得分差异有统计学意义(F=26.979,28.532,6.113,20.943;P<0.01);疫情的影响包括疫情对手术/治疗/复查的影响程度、对日常生活的影响程度、对家庭经济的影响程度在心理困扰得分差异有统计学意义(F=7.734,12.982,7.001;P<0.001);对疫情的担忧程度、消极应对方式、风险感知包括疾病易感性、疾病死亡率、疾病严重性在心理困扰得分差异有统计学意义(F=3.406,3.736,3.110,4.450,7.679;P<0.01);多元线性回归分析显示,疼痛、饮食状况、对疫情的担忧程度是心理困扰的重要影响因素。结论:新冠疫情下的肿瘤患者存在一定程度的心理困扰,疼痛、饮食状况、对疫情的担忧程度是心理困扰的重要影响因素。

Objective:To investigate the status of psychological distress and its influencing factors in tumor patients during the epidemic of novel coronavirus pneumonia.Methods:This cross-sectional descriptive study used convenience sampling to select 189 tumor patients from 5 tertiary hospitals in GuangDong province.The general information questionnaire,Kessler Psychological Distress Scale(K10)and Simple Coping Style Questionnaire(SCSQ)were used to conduct online questionnaire survey.Results:The detection rate of psychological distress was 50.79%.According to the one-way analysis of variance,body conditions including sleep status,diet status,weight status,and pain status(F=26.979,28.532,6.113,20.943;P<0.01)showed statistically significant differences in psychological distress scores;The impact of the epidemic included the degree of impact of the epidemic on surgery/treatment/review,the degree of impact on daily life,and the degree of impact on family economy(F=7.734,12.982,7.001;P<0.001);There were statistically significant differences in the scores of psychological distress in the degree of concern about the epidemic,negative coping style,risk perception including disease susceptibility,disease mortality,and disease severity(F=3.406,3.736,3.110,4.450,7.679;P<0.05).Multiple linear regression analysis showed that pain,diet status and anxiety about the epidemic were important factors affecting psychological distress.Conclusion:The tumor patients have certain degree of psychological distress during the epidemic of novel coronavirus pneumonia,which requires high attention and psychological support.
