某院首批国家药品带量采购实施效果评价与分析

Evaluation and Analysis of the Implementation Effect of the First Batch of the National Drug Procurement with Target Quantity Policy in a Hospital

目的评价上海市第十人民医院崇明分院实施首批国家组织药品集中采购(以下称为国家药品带量采购)政策的效果,为推进和完善我国药品集中采购模式提供参考。方法采集该院首批国家药品带量采购政策执行前后(2018年9月20日至2019年3月19日,2019年3月20日至2019年9月19日)的药物使用相关数据,回顾性分析执行前后的用药频度(DDDs)、DDDs变化幅度、限定日费用(DDC)、DDC变化幅度。结果首批国家药品带量采购涉及的18个品种中,DDDs降幅≥30%的有3个品种,DDDs变化幅度≤±10%的有3个品种(5个品规),DDDs增幅≥30%的有4个品种;DDC下降幅度≥50%的有12个品种,DDC上调的有3个品种。药品的DDC调整幅度与DDDs变化幅度逐步向量价挂钩方向发展,尚有部分药品未体现线性关系。结论首批国家药品带量采购政策的实施取得了一定效果,医疗活动中药品的价格重心整体下移,还需不断探索和完善政策的落地过程。

Objective To evaluate the effect of the first batch of the National Drug Centralized Procurement(National Drug Procurement with Target Quantity)Policy in Chongming Branch of Shanghai Tenth People’s Hospital,and to provide a reference for promoting the centralized drug procurement mode in China.Methods The data of drug use before the implementation(September 20,2018 to March19,2019)and after the implementation(March 20,2019 to September 19,2019)of the first batch of the National Drug Procurement with Target Quantily Policy in our hospital were collected,and the defined daily doses(DDDs),DDDs variation range,defined daily cost(DDC)and DDDc variation range before and after the implementation were analyzed retrospectively.Results Among the 18 varieties involved in the first batch of the National Drug Procurement with Target Quantity Policy,there were three varieties whose DDDs decreased≥30%,three varieties and five specifications whose DDDs changed≤±10%,three varieties(five specifications)whose DDDs increased≥30%,12 varieties whose DDC decreased≥50%,and three varieties whose DDC increased.The adjustment range of DDC and the change range of DDDs were gradually developing in the direction of hook quantity and price,and some drugs didn’t not reflect the linear relationship.Conclusion The implementation of the first batch of the National Drug Procurement with Target Quantity Policy has achieved a certain effect.The price focus of drugs in medical activities has moved down as a whole,and the implementation process of the policy still needs to be explored and improved.
