养阴和胃方对糖尿病性便秘患者胃肠动力的影响

Effect of Yangyin Hewei Fang on gastrointestinal motility in patients with diabetic constipation

目的探讨养阴和胃方治疗糖尿病性便秘患者的临床疗效以及对胃肠动力指标的影响。方法将100例糖尿病性便秘患者随机分为养阴和胃方组及对照组,各50例,分别予养阴和胃方及乳果糖口服,疗程4周。治疗结束后比较2组的便秘中医证候积分、口—结肠转运时间(OCTT)、血浆胃动素(MTL)、生长抑素(SS)及胃泌素(GAS)水平的变化。结果治疗结束,养阴和胃方组脱落2例,对照组无脱落。对照组总有效率(68%,34/50)低于养阴和胃方组总有效率(87.5%,42/48)(P<0.05)。治疗后,2组中医证候单项积分及总积分与治疗前比较均下降(P<0.05),且养阴和胃方组优于对照组(P<0.05);2组OCTT、MTL、SS及GAS均较治疗前改善,且养阴和胃方组优于对照组(P<0.001)。结论养阴和胃方能够有效改善糖尿病性便秘患者临床症状,改善胃肠动力,调整胃肠道相关激素分泌,疗效确切。

Objective To explore the clinical efficacy of Yangyin Hewei Fang in the treatment of diabetic constipation and its influence on gastrointestinal motility index.Methods One hundred patients with diabetic constipation were randomly divided into Yangyin Hewei Fang (YYHWF) group and control group,50 cases in each group.They were respectively treated with Yangyin Hewei Fang and lactulose orally for 4 weeks.After treatment,the changes in constipation TCM syndrome,oral-colon transit time (OCTT),plasma motilin (MTL),somatostain (SS) and gastrin (GAS) levels were compared between the two groups.Results At the end of the treatment,2 patients dropped out of the experiment in the YYHWF group,and none in the control group.The total effective rate of the control group (68%,34/50) was lower than that of the YYHWF group (87.5%,42/48) (P<0.05).After treatment,both the single and total scores of TCM syndrome in the two groups were decreased compared with those before treatment (P<0.05),and the YYHWF group was better than the control group (P<0.05).OCTT,MTL,SS and GAS in the two groups were improved compared with before treatment,and the YYHWF group was better than the control group (P<0.001).Conclusion Yangyin Hewei Fang can effectively improve the clinical symptoms of patients with diabetic constipation,improve gastrointestinal motility,and adjust the secretion of related hormones in the gastrointestinal tract,with a definite curative effect.
