摘要
目的比较应用超广角眼底成像系统与全视网膜镜周边部视网膜病变的检查效果。方法前瞻性病例对照研究。对郑州大学第一附属医院门诊2019年7月至2019年12月以眼前黑影飘动为主诉者1200例(2400只眼),分别应用自然瞳孔下超广角眼底成像系统检查及散瞳后全视网膜镜检查,比较两者的检查效果。结果超广角眼底成像系统的检查时间为(3.12±0.48)min,短于全视网膜镜的(8.53±2.15)min(t=-15.677,P<0.001)。前者的周边部视网膜病变检出率18.08%(434/2400),与后者的检出率19.25%(462/2400)接近。Kappa值>0.900,两种检查方法的结果有高度一致性。结论超广角眼底成像系统免散瞳、快捷、非接触,但不易观察上方及下方远周边部的细微病灶;全视网膜镜检查周边部检查详尽、更有立体感,但需要散瞳、接触、耗时较长。
Objective To compare the examination results of applying ultra-widefield fundus imaging system and panretinoscopy in peripheral retinopathy.Methods This was a prospective case-control study.A total of 1200 eyes of 1200 cases with dark shadow fluttering were examined from Jul.2019 to Dec.2019 in the First Affiliated Hospital of Zhengzhou University.All patients were examined with ultrawidefield fundus imaging system under natural pupil and panretinoscopy after pupil dilatation.The examination results were compared between the two groups.Results The average examination time of ultrawidefield fundus imaging system was(3.12±0.48)minutes,which was shorter than the(8.53±2.15)minutes of panretinoscopy cases(t=-15.677,P<0.001).The detection rate of peripheral retinal lesions with ultra-widefield fundus imaging system was 18.08%(434/2400),which was similar to the 19.25%(462/2400)of panretinoscopy.The detection rate was>0.900 after Kappa analysis,and the results of the two methods were highly consistent.Conclusion Ultra-widefield fundus imaging system is free from mydriasis,fast and with no contact.But it is difficult to observe the subtle lesions of superior and inferior retina in the far peripheral area.Panretinoscopy of peripheral retina is detailed with more third dimensional observations.But it requires pupil dilation and physical contact on the ocular surface,and it's timeconsuming.
