摘要
目的比较高强度聚焦超声与睫状体光凝术治疗绝对期青光眼的效果。方法回顾性临床研究。山西省眼科医院2018年1月至2018年5月就诊的45例(45眼)无光感的绝对期青光眼,患者根据手术方式分为两组。其中超声组,19例(19眼)行高强度聚焦超声治疗,光凝组,26例(26眼)接受睫状体光凝术。随访1个月,对两组术后眼压及疼痛评分进行比较。结果两组患者术前眼压均高达40 mmHg以上,组间比较差异无统计学意义(P=0.977)。术后1d、1周及1个月两组间眼压差异有统计学意义(P>0.05),术后高强度聚焦超声组眼压高于睫状体光凝组。术后各时间点平均眼压均低于术前(P<0.001)。超声组术后疼痛评分低于光凝组(Z=-2169,P=0.030)。结论治疗绝对期青光眼行高强度聚焦超声与睫状体光凝术均可降低眼压,超声治疗后患者疼痛较轻。
Objective To compare the efficacy between high intensity focused ultrasonotherapy andcyclophotocoagulation for the treatment of absolule glaucoma.Methods This was a retrospective clinicalstudy.A totle of 45 eyes of 45 cases of absolute glaucoma patients with no light perception from Jan.2018 to May 2018 in Shanxi Eye Hospital were divided into two groups,based on the surgical approach.The highintensity focused ultrasonotherapy group included 19 eyes of 19 cases and 26 eyes of 26 cases in thecyclophotocoagulation group.Postoperative intraocular pressure(IOP)and pain scores between the two groupswere compared during 1 month follow-up.ResultsThe IOP before surgery in two groups were higher than40 mmHg.There was no statistically sigficant difference between the two groups before surgery(P=0.977).The IOP at each time point after surgcry werc lower than that beforc surgery in the two groups(P>0.05)、Atl day,1 week and 1 month after surgery,the lOP in the ultrasound group were higher than that in thecyclophotocoagulation group(P<0.001).The 1OP of the two groups at each time point after surgery werelower than those before surgery(Pc0.001).Postoperative pain score of the ultrasound group was lower thanthat of the photocoagulation group(Z=-2.169,P=0.030).Conclusion Both high intensity focusedultrasonotherapy and cyclopholocoagulation for absolute glaucoma can reduce the intraocular pressure.Afterultrasound treament,the pain is mild.
