初始T1值和细胞外容积分数与弥漫性心肌纤维化的相关性:Meta分析 被引量:5

A Meta-analysis of correlation among native T1,extracellular volume fraction and diffuse myocardial fibrosis

摘要 目的:通过Meta分析系统评价T1 mapping定量指标[初始T1值和细胞外容积分数(ECV)]与弥漫性心肌纤维化(DMF)程度的相关性。方法:系统检索2019年9月以前发表于PubMed、Cochrane Library、Medline、CNKI、维普、万方数据库中关于初始T1值和ECV与DMF程度相关性的中英文文献。采用诊断准确性试验质量评价工具2(QUADAS-2)对纳入文献进行质量评价,并应用Stata 15.0软件对纳入文献的r值进行统计学分析。结果:最终纳入11篇文献。初始T1值与DMF程度呈中度正相关(r=0.54,P=0.00,95%CI:0.41~0.65),ECV与DMF程度呈强正相关(r=0.62,P=0.00,95%CI:0.54~0.69)。结论:初始T1值和ECV与DMF程度存在较好的相关性,T1 mapping可早期无创评估心肌纤维化,指导临床诊疗。 Objective:To systematically evaluate the correlation between T1 mapping quantitative indexes[native T1 and extracellular volume fraction(ECV)]and the degree of diffuse myocardial fibrosis(DMF)by using Meta-analysis.Method:Chinese and English literatures involving correlations between T1 mapping quantitative indexes(native T1 and ECV)and DMF published in PubMed,Cochrane Library,Medline,CNKI,VIP,and Wanfang database before September 2019 were searched systematically.QUADAS-2 was used to evaluate the quality of the included literatures.The correlation coefficient r value of the included literatures was statistically analyzed by Stata 15.0 software.Result:Totally,11 studies were enrolled.Native T1 was moderately correlated with the degree of DMF(r=0.54,P=0.00,95%CI:0.41-0.65),while ECV was strongly correlated with the degree of DMF(r=0.62,P=0.00,95%CI:0.54-0.69).Conclusion:Native T1 and ECV have a good correlation with the degree of DMF,and T1 mapping can be used to noninvasively evaluate myocardial fibrosis in the early stage and guide clinical diagnosis and treatment.
