摘要
目的探究耳后隐蔽切口入路对腮腺肿瘤切除术患者面神经功能的疗效影响。方法选取腮腺肿瘤切除术患者76例,随机分为观察组和对照组,各38例。对照组采用传统S型切口入路,观察组采用耳后隐蔽切口入路。比较两组患者围手术期基本情况、手术前及术后1周时炎症因子、手术前及术后3个月时面神经功能、术后3个月时两组患者并发症发生率等指标的差异。结果观察组手术时间显著高于对照组,术中出血量、住院时间均显著低于对照组;术后1周时,两组患者TNF-α、IL-6指标均较手术前显著上升,且观察组指标明显低于同一时间对照组;术后3个月时两组患者SFGS评分均较手术前显著下降,且观察组评分显著高于同一时间对照组;术后3个月时,观察组并发症发生率低于对照组,但差异不显著。结论耳后隐蔽切口入路术较传统手术方式时间略长,但治疗效果明确,可显著预防术后感染,对腮腺肿瘤患者面部功能影响较小。
Objective To investigate the efficacy of concealed posterior auricular incision approach on facial nerve function in patients with resection of parotid gland tumor.Methods Seventy-six patients with resection of parotid gland tumor were selected.According to the random number table method,the patients were divided into observation group and control group,with 38 cases in each group.Control group was given traditional S-shaped incision approach while observation group was given concealed posterior auricular incision approach.The perioperative basic conditions were compared between the two groups.The inflammatory factors were analyzed in the two groups before surgery and at 1 week after surgery.The facial nerve function was observed before surgery and at 3 months after surgery.The difference of occurrence of complications was compared between the two groups at 3 months after surgery.Results The surgery time,intraoperative blood loss and hospital stay in observation group were significantly lower than those in control group(P<0.05).At 1 week after surgery,the levels of TNF-αand IL-6 in the two groups were significantly increased compared with those before surgery,and the levels in observation group were significantly lower than those in control group at the same time(P<0.05).At 3 months after surgery,the SFGS scores in the two groups were significantly decreased compared with those before surgery,and the scores in observation group were significantly higher than those in control group at the same time.At 3 months after surgery,the incidence rate of complications in observation group was lower than that in control group,but the difference was not significant.Conclusion Compared with traditional S-shaped incision approach,concealed posterior auricular incision approach for resection of parotid gland tumor has less time-consuming and clearer treatment effects,and it can significantly prevent postoperative infection and has less effect on facial function of patients with parotid gland tumor.
