温针配合药物治疗心肾阳虚型不稳定型心绞痛疗效观察

Efficacy Observation of Warm Needling Combined with Medication for Unstable Angina Pectoris ofHeart-kidney Yang Deficiency Pattern

目的观察温针配合药物治疗心肾阳虚型不稳定型心绞痛的临床疗效。方法将80例心肾阳虚型不稳定型心绞痛患者随机分为治疗组和对照组,每组40例。治疗组采用温针配合药物治疗,对照组采用单纯药物治疗。观察两组治疗前后心绞痛发作情况(发作频率、持续时间)、心功能指标[左心射血分数(LVEF)、每搏量(SV)]及血清内皮细胞特异分子-1(ESM-1)、细胞间黏附分子-1(ICAM-1)水平的变化情况,并比较两组临床疗效及心绞痛分级。结果治疗组总有效率为95.0%,对照组为75.0%,两组比较差异具有统计学意义(P<0.05)。两组治疗后心绞痛发作频率、持续时间、心功能指标及血清ESM-1、ICAM-1水平与同组治疗前比较差异均具有统计学意义(P<0.05)。治疗组治疗后心绞痛分级、心绞痛发作频率、持续时间、心功能指标及血清ESM-1、ICAM-1水平与对照组比较差异均具有统计学意义(P<0.05)。结论温针配合药物治疗心肾阳虚型不稳定型心绞痛疗效确切,可减少心绞痛发作频次及持续时间,改善心绞痛分级及心功能指标,可能与其降低血清ESM-1、ICAM-1水平有关。

Objective To observe the clinical efficacy of warm needling plus medication in treating unstable angina pectoris(UAP)of heart-kidney yang deficiency pattern.Method Eighty patients with UAP of heart-kidney yang deficiency pattern were randomized into a treatment group and a control group,with 40 cases in each group.The treatment group was intervened by warm needling plus medication,while the control group was treated with medication alone.Angina pectoris attack parameters(attack frequency and duration),cardiac function indexes[left ventricular ejection fraction(LVEF)and stroke volume(SV)],and the levels of serum endothelial cell specific molecule-1(ESM-1)and intercellular adhesion molecule-1(ICAM-1)were observed before and after treatment in the two groups.The clinical efficacy and angina pectoris grade were also compared between the two groups.Result The total effective rate was 95.0%in the treatment group,versus 75.0%in the control group,and the between-group difference was statistically significant(P<0.05).The attack frequency and duration of angina pectoris,cardiac function indexes and serum ESM-1 and ICAM-1 levels changed significantly after treatment in both groups(P<0.05).After treatment,the angina pectoris grade,attack frequency and duration of angina pectoris,cardiac function indexes,and serum ESM-1 and ICAM-1 levels in the treatment group were significantly different from those in the control group(P<0.05).Conclusion Warm needling plus medication is effective in treating UAP of heart-kidney yang deficiency pattern;it can reduce the attack frequency and duration of angina pectoris,and improve angina pectoris grade and cardiac function indexes,which may be associated with the down-regulation of serum ESM-1 and ICAM-1 levels.
