责任制整体护理对肺结核患者自护能力及服药依从性的影响

The Influence of Responsibility System Holistic Nursing on the Self-care Ability and Medication Compliance of Pulmonary Tuberculosis Patients

目的研究责任制整体护理对肺结核患者自护能力及服药依从率的影响。方法选取2019年1月~2020年1月在我院诊治的94例肺结核患者为研究对象,采用随机数字表法分为对照组和观察组,各47例。对照组采用常规护理,观察组实施责任制整体护理干预,比较两组疾病控制时间、住院时间、自护能力(自护技能、自护责任感、自我概念、疾病健康知识)评分、服药依从率、生活质量(SF-36)评分、护理满意度以及复发率。结果观察组疾病控制时间、住院时间均短于对照组,差异有统计学意义(P<0.05);干预后观察组自护技能、自护责任感、自我概念、疾病健康知识评分均高于对照组,差异有统计学意义(P<0.05);观察组服药依从率为93.61%,高于对照组的82.97%,差异有统计学意义(P<0.05);干预后观察组SF-36生活质各维度量评分均高于对照组,差异有统计学意义(P<0.05);观察组护理满意度为95.74%,高于对照组的85.10%,差异有统计学意义(P<0.05);随访3个月,观察组复发率为8.51%,低于对照组的19.14%,差异有统计学意义(P<0.05)。结论责任制整体护理可提高肺结核患者自护能力和服药依从率,缩短患者疾病控制和住院时间,改善患者生活质量,提升护理满意度,降低复发率,具有确切的应用有效性和可行性。

Objective To study the effect of responsible holistic care on the self-care ability and medication compliance rate of tuberculosis patients.Methods A total of 94 pulmonary tuberculosis patients diagnosed and treated in our hospital from January 2019 to January 2020 were selected as the research objects,and were divided into control group and observation group by random number table method,with 47 cases in each group.The control group adopts routine nursing,and the observation group implements the responsibility system of overall nursing intervention.Compare the two groups'disease control time,hospital stay,self-care ability(self-care skills,self-care responsibility,self-concept,disease and health knowledge)score,medication compliance rate,quality of life(SF-36)score,nursing satisfaction,and recurrence rate.Results The disease control time and hospital stay of the observation group were shorter than those of the control group,the difference was statistically significant(P<0.05);After the intervention,the observation group's self-care skills,self-care responsibility,self-concept,and disease health knowledge scores were higher than those of the control group,the difference was statistically significant(P<0.05);The medication compliance rate of the observation group was 93.61%,which was higher than 82.97%of the control group,the difference was statistically significant(P<0.05);After the intervention,the scores of all dimensions of SF-36 quality of life in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05);The nursing satisfaction degree of the observation group was 95.74%,which was higher than 85.10%of the control group,the difference was statistically significant(P<0.05);After 3 months of follow-up,the recurrence rate of the observation group was 8.51%,which was lower than 19.14%of the control group,the difference was statistically significant(P<0.05).Conclusion Responsible holistic care can improve the self-care ability and medication compliance rate of tuberculosis patients,shorten the patient's disease control and hospital stay,improve the quality of life of patients,increase nursing satisfaction,and reduce the recurrence rate.It has definite application effectiveness and feasibility.
