外周血CD4^(+)CD69^(+)T细胞在AIHA患者中的表达及与疾病严重程度的相关性

Expression of CD4^(+)CD69^(+) T cells in peripheral blood of patients with AIHA and its correlation with disease severity

目的:分析外周血CD4^(+)CD69^(+) T淋巴细胞在自身免疫性溶血性贫血(AIHA)患者中的表达及与疾病严重程度的相关性。方法:回顾性分析42例AIHA患者的临床资料,包括缓解24例,溶血发作18例;另以25例体检健康人员为对照组。采用流式细胞仪检测外周血CD69^(+) T淋巴细胞,酶联免疫吸附试验法检测血浆CD69水平,实时荧光定量聚合酶链反应检测外周血CD4^(+)T淋巴细胞CD69 mRNA的表达情况。结果:与对照组比较,溶血组CD3^(+)CD69^(+)和CD3^(+)CD4^(+)CD69^(+)T淋巴细胞绝对值及CD3^(+)CD8^(+)CD69^(+)/CD3^(+)CD8^(+)T淋巴细胞比值均明显升高(P<0.05);与对照组和缓解组比较,溶血组CD3^(+)CD69^(+)/CD3^(+)和CD3^(+)CD4^(+)CD69^(+)/CD3^(+)CD4^(+)T淋巴细胞比值均明显升高(P<0.05)。AIHA患者CD3^(+)CD8^(+)CD69^(+)T淋巴细胞绝对值与对照组比较,差异无统计学意义(P>0.05)。与对照组,溶血组比较血浆CD69水平明显升高(P<0.05)。与对照组和缓解组比较,溶血组外周血CD4^(+)T淋巴细胞CD69 mRNA表达量明显升高(P<0.05)。结论:AIHA患者外周血CD69^(+) T淋巴细胞比值明显高于正常人群,尤其是外周血CD3^(+)CD4^(+)CD69^(+) T淋巴细胞升高,且与AIHA患者病情严重程度存在密切关系。

Objective:To analyze the expression of CD4^(+)CD69^(+) T lymphocytes in peripheral blood of patients with autoimmune hemolytic anemia(AIHA)and the correlation between the expression and the severity of the disease. Method:The clinical data of 42 patients with AIHA in our hospital were analyzed retrospectively,including 24 cases of remission,18 cases of hemolytic attack,and 25 healthy persons as the control group.Flow cytometry was used to detect CD69^(+) T lymphocytes in peripheral blood,enzyme-linked immunosorbent assay was used to detect the level of plasma CD69,and real-time fluorescence quantitative PCR was used to detect the expression of CD69 mRNA in peripheral blood CD4^(+) T lymphocytes. Result:Compared with the control group,the absolute value of CD3^(+)CD69^(+) and CD3^(+)CD4^(+)CD69^(+) T lymphocytes and the ratio of CD3^(+)CD8^(+)CD69^(+)/CD3^(+)CD8^(+) T lymphocytes in the heamolytic attack group were significantly increased(P<0.05);compared with the control group and the remission group,the ratio of CD3^(+)CD69^(+)/CD3^(+) and CD3^(+)CD4^(+)CD69^(+)/CD3^(+)CD4^(+) T lymphocytes in the hemolytic attack group were significantly increased(P<0.05).There was no significant difference in the absolute value of CD3^(+)CD8^(+)CD69^(+) T lymphocytes between AIHA and control group(P>0.05).Compared with the control group,the level of plasma CD69 in hemolytic attack group was significantly increased(P<0.05).Compared with the control group and the remission group,the expression of CD69 mRNA in peripheral blood CD4^(+) T lymphocytes in hemolytic attack group was significantly increased(P<0.05).Conclusion:The ratio of peripheral blood CD69^(+) T lymphocytes in patients with AIHA is significantly higher than that in normal people,especially the increase of peripheral blood CD3^(+)CD4^(+)CD69^(+) T lymphocytes,which is closely related to the severity of AIHA.
