双平板DSA辅助防护设施改进及屏蔽效果评价研究 被引量:2

摘要 目的测量和分析双平板DSA术者位平面的辐射剂量水平,改进辅助防护设施,为使用双平板DSA的介入职业人员提供合理可行的放射防护建议。方法设计由移动主屏风和悬挂侧屏风组成的辅助防护设施。双球管同时曝光,使用AT1123辐射剂量监测仪测量第一、第二和第三术者(分别位距离球管60、120、180 cm)位的辐射剂量,每个术者位平面选择距地20~180 cm范围内九个检测点,分析比较辅助防护设施改进前后的防护效果。结果辅助防护设施改进后,介入职业人员腰部以上位置(距地105 cm及以上)屏蔽效率为37.5%~96.2%;其中第一术者和第二术者腰部以上位置的屏蔽效率大于60%;第一、第二、第三术者位平面的辐射剂量水平均降低至WS 76-2020标准限值(≤400μSv/h)以下;与标配防护下的数据相比,第一、第二、第三术者位平面的辐射剂量均具有统计学差异(P<0.001),屏蔽效果显著。结论通过改进辅助防护设施,可明显降低介入职业人员外照射剂量水平,有效保障介入职业人员的辐射安全和职业健康。 Objective To provide reasonable and feasible radiation protection recommendations for intervention professionals who use bi-plane digital subtractive angiography(DSA),by analyzing the radiation dose level of the intervention physicians(surgeons)under different auxiliary radiological protective facilities.Methods Designed by the main mobile screen and hanging side screen of the auxiliary protection facilities,an AT1123 radiation dose monitor was used to measure the radiation dose rate in the plane of the 1^(st),2^(nd) and 3^(rd) surgeons(The distance away from the main tube is 60 cm,120 cm and 180 cm respectively)when dual x-ray tubes expose simultaneously.Nine detection points were selected at 20-180 cm height above the ground in each plane.The protective effects before and after the improvement of auxiliary radiological protective facilities were evaluated.Results After the improvement of auxiliary radiological protective facilities,the shielding efficiency of intervention professionals above the waist level was 37.5%~96.2%.Especially for the first and second surgeon,the shielding efficiency at 105 cm and above was greater than 60%.The radiation dose level on the planes of the position of intervention professionals was less than WS 76-2020 standard limits(400μSv/h).Compared with the radiation dose level under the default protective facilities,the shielding effect was statistically significant(P<0.001).Conclusion After the improvement of auxiliary radiological protective facilities,the external exposure dose of intervention professionals can be significantly reduced,which effectively guarantee the radiation safety and health of the personnel involved in the occupation.
