从体证相关角度探讨间质性肺疾病

Discussion About Interstitial Lung Disease from the Perspective of Body Syndrome Correlation

目的:探究间质性肺疾病(interstitial lung disease,ILD)体质与中医证型的相关性。方法:制定ILD患者中医体质与证型临床观察表,分别对ILD患者的体质及证型进行判断,建立Excel数据库并运用SPSS 21.0软件进行统计分析。结果:198例ILD中男125例,女73例,男女比例为1.71:1;病程(3.34±2.30)年;中医体质以阴虚质、痰湿质、气虚质、血瘀质最为常见;证型分布以阴虚内热证、痰热壅肺证、肺肾气虚证、痰瘀阻肺证和肺肾气阴两虚证为主。阴虚质患者中,阴虚内热证最多,其次是肺肾气阴两虚证;痰湿质患者中,痰热壅肺证最多,其次是痰瘀阻肺证;血瘀质患者中,痰瘀阻肺证最多。结论:ILD患者体质分布以阴虚质和痰湿质为主,证型以阴虚内热证和痰热壅肺证为主,且两者之间有相关性。

Objective:To explore the correlation between constitution and TCM syndrome types of interstitial lung disease(ILD).Methods:The clinical observation table of TCM constitution and syndrome types of ILD patients was made,and the constitution and syndrome types of ILD patients were judged respectively.Excel database was established and SPSS21.0 software was used for statistical analysis.Results:Among 198 cases of ILD,there were 125 males and 73 females,The ratio of male to female was 1.71:1;The course of disease was(3.34±2.30)years.The most common TCM constitutions included yin deficiency,phlegm dampness,qi deficiency and blood stasis;The main syndrome types were as follows:yin deficiency and internal heat syndrome,phlegm-heat obstructing lung syndrome,lung and kidney qi deficiency syndrome,stagnant phlegm obstructing lung syndrome and lung and kidney qi and yin deficiency syndrome.Among the patients with yin deficiency,the syndrome of yin deficiency and internal heat was the most,followed by the syndrome of lung kidney qi and yin deficiency.Among the patients with phlegm and dampness,phlegm-heat obstructing lung syndrome was the most common,followed by stagnant phlegm obstructing lung syndrome.Among the patients with blood stasis syndrome,the syndrome of stagnant phlegm obstructing the lung was the most.Conclusion:The constitution distribution of ILD patients is mainly yin deficiency and phlegm dampness,and the syndrome types are mainly yin deficiency internal heat syndrome and phlegm-heat obstructing lung syndrome,and there is a correlation between them.
